    【】目的:评价时间一空间相关成像 (STIC) 技术在妊娠期糖尿病胎儿左心室容积及左心室收缩功能的临床应用。方法:运用时间一空间相关成像技术采集120例正常孕妇胎儿及46例妊娠期糖尿病孕妇胎儿心动周期图像,通过VOCAL功能获得容积参数,包括左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV),计算出左心室每搏心输出量(LVSV)、左心室射血分数(LVEF),左心室心输出量(LVCO)。将妊娠期糖尿病(GDM)分为两组:血糖控制良好组(GDMI组):空腹血糖<5.8 mmol/I,经饮食控制,餐后2 h血糖<6.7mmol/L;血糖控制不佳组(GDMII组):空腹血糖≥5.8 mmol/L,或者经饮食控制,餐后2h血糖≥6.7 mmol/L,需加用胰岛素。结果:(1)与对照组相比,GDMI组胎儿LVSV 、LVEF、LVCO差异无统计学意义(P>0.05) ,GDMI组胎儿的左室LVEDV、LVESV减小,差异有统计学意义(P<0.05);(2)与对照组相比,GDMII组胎儿的LVEDV、LVESV、LVSV、LVEF、LVCO均增大,差异有统计学意义(P<0.05);(3)与GDMI组相比,GDMII组胎儿的LVEDV、LVESV、LVSV、LVEF、LVCO均增大,差异有统计学意义(P<0.05)。结论:时间一空间相关成像可以有效评估妊娠期糖尿病胎儿左心容积变化,进而评估胎儿左心整体收缩功能。

    Abstract:

    Objective: To investigate the value of Spatio-temporal Image Correlation(STIC) technique in evaluating the left fetal ventrlenlar volume and systolic function in pregnant women with gestational diabetes mellitus(GDM).Methods:Fetal heart Ventricular volumes were measured using STIC technique in 46 women with GDM(GDM group)and 120 women with normal pregnancy(normal group).The virtual organ computer—aided analysis(VOCAL)technique was used to obtain left systolic and diastolic ventricular volumes.The stroke volume(LVSV),ejection fraction (LVEF)and cardiac output(LVCO) were calculated.Patients with gestational diabetes mellitus were divided into 2 groups:GDMI group: The plasma levels of FPG is less than 5.8 mmol/L, PPG is less than 6.7mmol/L; GDMII group: The plasma levels of FPG is higher than 5.8 mmol/L, PPG is higher than 6.7mmol/L. Results:Compared With normal,the left systolic and diastolic ventricular volumes of GDMI group was decreased (P<0.05); the left systolic and diastolic ventricular volumes and the left stroke volume(LVSV),ejection fraction(LVEF)and cardiac output(LVCO)of GDMII group was higher(P<0.05); Compared With GDMI group, the left systolic and diastolic ventricular volumes and the left stroke volume(LVSV),ejection fraction(LVEF)and cardiac output(LVCO)of GDMII group was higher(P<0.05); Conclusions: STIC is an adoptable reliable and noninvasive investigation of left fetal ventricular volume in pregnant women with gestational diabetes mellitus ,then can evaluate the systolic function.
