【】 目的:研究分析不同PSA区间内超声造影(CEUS)、磁共振增强(MRI增强)及其联合成像在前列腺癌诊断中临床应用价值。方法:回顾性分析132例疑似前列腺癌患者,均行CEUS、MRI增强检查,并进一步经直肠前列腺穿刺活检术证实,结合病理结果比较分析不同PSA区间内两种诊断方法及其联合成像在前列腺癌诊断中临床应用价值。结果:在4.0ng/ml≤PSA<10.0ng/ml范围内,在4例前列腺癌患者中,CEUS 、MRI增强及二者联合应用诊断正确分别为2例、3例和4例,CEUS、MRI增强及二者联合应用特异度分别为82.00%、96.00%、80.00%,MRI增强与CEUS及MRI增强与二者联合应用诊断前列腺癌的特异度差异均有统计学意义(p<0.05)。在10.0ng/ml≤PSA<20.0ng/ml范围内,CEUS、MRI增强及二者联合应用对前列腺癌诊断的灵敏度分别为42.86%、52.38%、80.95%,特异度分别为73.68%、68.42%、63.16%,二者联合应用与单独一种技术的诊断灵敏度差异均具有统计学意义(p<0.05)。在PSA≥20.0ng/ml中,32例前列腺癌,三种方法均诊断正确。结论:在4.0ng/ml≤PSA<10.0ng/m区间,MRI增强具有较高的诊断特异性,有诊断优势;10.0ng/ml≤PSA<20.0ng/ml中,CEUS和MRI增强联合应用具有较高的灵敏度,具有诊断优势;在PSA≥20.0ng/ml中CEUS检查简便,可提高前列腺穿刺活检定位准确性,具有诊断优势。 |

【】Objective To explore the value of contrast-enchanced ultrasound(CEUS) with enhanced magnetic resonance imaging(enhanced MRI) in different area of PSA for prostate cancer detection. Methods 132 patients suspected prostate cancer patients were retrospectively analyzed and all the patients underwent CEUS and enhanced MRI. And all patients further performed prostate biopsy through transrectal for prostate cancer final diagnosis. The result pathological of biopsy were performed to evaluate the diagnostic value of CEUS and enhanced MRI in different area of PSA. Result When PSA in the range of 4.0ng/ml to 10.0ng/ml, in 4 cases of prostate cancer, CEUS, MRI enhancement and combined diagnosis were diagnosed correctly by 2 cases, 3 cases and 4 cases respectively. The specificity of CEUS, MRI and the combined application were 82%, 96%, 80% respectively. There were significant differences compare MRI enhancement and combined diagnosis in diagnosis of prostate cancer(p<0.05). When PSA in the range of 10.0ng/ml to 20.0ng/ml The sensitivity of CEUS, MRI and the combined application to the diagnosis of prostate cancer were 42.86%, 52.38% and 80.95% respectively, and the specificity was 73.68%, 68.42% and 63.16% respectively. When PSA more than 20.0ng/ml, in 32 cases of prostate cancer were all diagnosed correctly with three kinds of methods. Conclusion When PSA in the range of 4.0ng/ml to 10.0ng/ml, MRI enhanced diagnosis with high specificity. So the MRI enhanced has diagnostic advantage. When PSA in the range of 10.0ng/ml to 20.0ng/ml, the combined application of CEUS and MRI has high sensitivity. The combined application has the advantage of diagnosis. When PSA more than 20.0ng/ml, Simple CEUS check can improve the positioning accuracy of prostate biopsy. So CEUS diagnosis has advantages. |
