【】目的 应用彩色多普勒血流成像技术(color Doppler flow imaging,CDFI)评估左侧锁骨下动脉起始段支架植入术后血流动力学改变、术后再狭窄发生率、后期转归及相关危险因数的影响。方法 取本院自2010年2月-2012年8月左侧锁骨下动脉起始段支架置入术后患者58例,于术后1个月、3个月、6个月、12个月、24个月、36个月复查,个别患者增加了复查次数,分别对比术前与术后1个月狭窄段管径、狭窄段及远心段肱动脉的血流参数收缩期峰值流速(PSV)。统计各时段支架术后的再狭窄、闭塞等情况的发生率,分析导致术后再狭窄的各种危险因素。结果 CDFI显示支架置入术后1个月狭窄管径由术前(3.20±2.20)mm增加至(8.50±3.60)mm(P=0.01),狭窄处PSV由术前的(325±113)cm/s下降为(158±102)cm/s,对应的左侧肱动脉血流PSV由术前的(45±17)cm/s变为(75±23)cm/s。CDFI随访1-36个月支架的再狭窄率分别为1个月5.1%、3个月15.5%、6个月27.5%、12个月34.4%、24个月36.2%、36个月37.9%。结论 CDFI可以较好的评估左侧锁骨下动脉支架术的疗效,可以及时的发现支架的再狭窄,在术后评估中具有重要的意义。 |

【】Objective To evaluate and follow-up the effection of left subclavian artery origin stenting(SAOS)by color Doppler flow imaging (CDFI) and assess the rate of restenosis after stenting,hemodynamic changes after stent implantation, prognosis and correlative risk factors of influence.Methods 58 cases of left subclavian artery origin stenting in patients in my hospital from 2010 June to 2012 August.The diameter of SAOS, peak systolic velocity(PSV) of segment at origin and brachial artery were evlaueted before and every 1、3、6、12、24、36month after SAOS by CDFI.Results the diameter of SAOS was improved from (3.20 2.20) mm to (8.50 3.60) mm (P=0.02), PSV of segment at origin and brachial artery also decreased from(325±113)cm/s to(158±102)cm/s (P=0.01)and increased from(45±17)cm/s to(75±23)cm/s (P=0.01).range 1 to 36 months,the cumulative restenosis rate at 3、6、12、24、36 month were5.1%、15.5%、27.5% 、34.4%、36.2%、37.9%。Conclusions CDFI is a sensitive and noninvasive examination to follow-up SAOS and detect in-stent restenosis immediately. the left subclavian artery initial segment of in stent restenosis reference standard should improve the preoperative; residual stenosis、 vascular bending、infection、 stoping drug or medication is not law and other factors can increase the rate of restenosis. |
