目的:探讨声辐射力脉冲成像(ARFI)技术在定量评价不同类型颈动脉粥样斑块硬度的价值。方法:应用二维超声特点,将141个斑块分为软斑块组、混合斑块组和硬斑块组,测定不同类型斑块的剪切波传播速度(SWV)、斑块厚度及血脂水平,进行对比分析。结果:三组不同类型的斑块,其SWV差异有统计学意义(P<0.0001),软斑块组最慢,其次是混合斑块组,硬斑块组最快,ROC曲线分析,诊断混合性斑块和硬斑块的临界值分别为1.57m/s、2.14m/s。结论:ARFI技术对判断粥样斑块的稳定性能提供较为可靠的定量指标,具备一定的临床意义。 |

Abstract: |
Objective to explore the diagnostic value of ARFI in the quantitativeSevaluation of different types of scirrhosity of carotid plaques. Method based on two-dimensional ultrasound, to divide 141 carotid plaques into soft-plaque,Shard-plaqueSand mixed-plaque groups, then to detect shear wave velocity (SWV), plaqueSthickness and blood lipid levelsSof different types of carotid plaques, and finally to make a comparative analysis. Result the SWV of three different types of carotid plaques has a statistical significance(P<0.0001), soft-plaque is the slowest, mixed-plaque is in the middle, and hard-plaque is fastest. The critical value of mixed-plaque and hard-plaque is 1.57m/s and 2.14m/s by ROC analysis respectively. Conclusion ARFI is a reliable quantitative index to judge the stability of carotid plaques, which is helpful in clinical significance. |
