【】目的 探讨斑点追踪技术评估室间隔缺损(VSD)患儿左室扭转运动的临床价值。方法 随机选取室间隔缺损(VSD)患儿(36例),应用实时三维超声分析获得左室舒张末期容积及收缩末期容积,软件自动计算左室射血分数,应用二维斑点追踪技术测量并记录两组左室扭转运动参数,探讨斑点追踪技术评估VSD患儿左室扭转运动的临床价值。结果 相较于对照组,VSD组左室基底部旋转角度、左室基底部旋转角度峰值、左室扭转角度及扭力均明显大于对照组(P<0.01),心尖部旋转角度及心尖部旋转角度峰值与对照组无统计学意义(P=0.525、0.757)。左室扭转运动参数与左室舒张末期容积及左室舒张末期容积指数无明显相关性。结论 VSD导致的左心容量负荷增加对左室整体扭转运动及基底部旋转运动有影响,对左室心尖部旋转运动影响不明显,左室扭转运动参数与左室容积无明显相关性。 |

【】Objective To evaluate the clinical value of speckle tracking technique in evaluating the left ventricular twist in children with ventricular septal defect(VSD). Methods Patients with ventricular septal defect (36 cases) as the case group,age and sex matched normal children(36 cases) as the control group,left ventricular end diastolic volumeand end systolic volume and Left ventricular ejection fraction were obtained by real-time three-dimensional echocardiography,two dimensional speckle tracking technique was used to measure the left ventricular twist motion parameters of the two groups to evaluate the clinical value of speckle tracking technique in evaluating the left ventricular twist in children with VSD. Results In the VSD group,the left ventricular basal rotation angle,peak of the left ventricular basal rotation angle,peak of the left ventricular twist angle and torque were significantly greater than that in the control group (P < 0.01) while the apical rotation and peak of the apical rotation angle had no statistical significance (P=0.525、0.757) with the control group.Also,there was no significant correlation between left ventricular twist motion parameters and 3DQA-EDV and 3DQA-EDVI. Conclusions The overload of the left ventricular volume in VSD increase the left ventricular global and basal rotation,the left ventricular twist motion parameters and left ventricular volume had no significant correlation. |
