目的:探讨经会阴四维超声在诊断女性压力性尿失禁(SUI)中的应用价值。方法:应用经会阴四维超声,以32例正常经产妇为对照,分别在正中矢状面和三维重建面观察39例SUI患者在静息和Valsalva动作两种状态下四维超声图像的变化,并分析平静呼吸状态下的尿道长度、膀胱逼尿肌厚度,测量静息状态及最大Valsalva动作下的盆膈裂孔面积、膀胱颈至耻骨联合后下缘的垂直距离、膀胱尿道后角、尿道旋转角,计算膀胱颈移动度。结果:静息时,SUI组膀胱颈至耻骨联合下缘垂直距离和对照组比较,差异无统计学意义(P=0.426)。张力状态下SUI组膀胱颈至耻骨联合下缘垂直距离、膀胱尿道后角及盆膈裂孔面积与对照组之间差异均有统计学意义(P<0.05)。SUI组的膀胱颈移动度及尿道旋转角均显著高于对照组,两组差异有统计学意义(P<0.05)。结论:经会阴四维超声可动态评估女性压力性尿失禁患者的盆底解剖和功能,值得临床推广应用。 |

Abstract: |
Objective To investigate the application value of transperineal four-dimensional ultrasound in female patients with stress urinary incontinence.
Methods Transperineal four-dimensional ultrasound was performed in 32 cases of normal women and 39 cases of female patients with stress urinary in continence. In condition of resting and tension, the length of urethra and the thickness of the bladder detrusor were observed in the resting condition; the pelvic diaphragm hiatal area,the vertical distance from vesical neck to the edge of the pubic symphysis, posterior angle of bladder urethral, urethral rotation angle were measured in the resting and tension condition; The bladder neck movement was calculated.
Results The vertical distance from vesical neck to the edge of the pubic symphysis showed no statistical difference between SUI group and control group (P=0.426) in resting condition; the vertical distance from vesical neck to the edge of the pubic symphysis, posterior angle of bladder urethra and the pelvic diaphragm hiatal area of SUI group were significantly greater than the control group (P<0.05). The bladder neck movement and urethral rotation angle were significantly higher than the control group (P<0.05).
Conclusions The transperineal four-dimensional ultrasound can dynamically evaluated female stress urinary incontinence in patients with pelvic floor anatomy and function, and is worthy of clinical application. |
