目的 探讨时间强度曲线(TIC)对甲状腺结节超声造影的鉴别诊断价值 方法 对163例患者共167枚甲状腺结节术前行超声造影检查。经手术病理证实结节性甲状腺肿30例,腺瘤37例,桥本甲状腺炎6例,乳头状癌94例。分别绘制结节ROI和实质ROI的TIC,记录TIC形态和各造影参数峰值强度(Peak)、达峰时间(TtoPk)、及曲线下面积(Area)等,并计算结节ROI减去实质ROI的各超声造影参数△Peak、△Area等,分析不同结节大小(≤1cm和>1cm)TIC形态和定量参数差异 结果 良恶性结节在增强相和消退相表现均有统计学差异(P<0.001)。结节性甲状腺肿多表现为同进同退,腺瘤多表现为快进快退,微小乳头状癌(≤1cm)多表现为慢进快退,甲状腺乳头状癌(>1cm)多表现为快进快退。>1cm的结节,良性结节的Peak和△Area高于恶性结节,差异有统计学意义(P<0.05);对于<1cm的结节,良性结节Area、Peak和△Peak高于恶性结节,差异均有统计学差异(P<0.05)。结论 超声造影结合TIC曲线分析甲状腺结节微循环灌注情况,对甲状腺良恶性结节鉴别诊断有一定意义。 |

Objective To investigate the application value for time intensity curve(TIC)in contrast enhanced ultrasound(CEUS)in the differential diagnosis of thyroid nodules(TNs).Methods 163 patients with 167 TNs (62 papillary thyroid carcinomas, 30 nodular goiters, 6 Hashimoto thyroiditis and 30 adenomas) that underwent contrast-enhanced sonography with a linear probe (6-9 MHz, LOGIQE9/GE) were included in this study. Time intensity curve of TNs and the adjacent thyroid parenchyma were acquired with dedicated software.The shape and various parameters were recorded including peak intensity (Peak), time to peak (TtoPk), the area under the curve (Area), etc. Then calculate the different parameter values between TNs and the adjacent thyroid parenchyma such as △Peak, △Area and so on.The lesions were divided into group 1(≤1)and group 2(>1 cm)by maximum diameter.The shape of TIC and quantitative analysis findings of CEUS of nodules were compared.Results Benign and malignant TNs show different in wash-in and wash-out phase (P<0.001) . Nodular goiters have identical-in and identical-out. Adenomas have earlyh-in and early-out. Papillary thyroid microcarcinomas have slow-in and early-out.While papillary thyroid carcinomas (>1 cm) have early-in and early-out. Peak and Area in benign TNs were higher than in maligant ones among group 1.(P<0.05) Area, Peak and△Peak in benign TNs were higher than in maligant ones among group 2.(P<0.05)Conclusion CEUS in combination with TIC analysis allows an evaluation of the microvascularisation of TNs and is helpful for the differentiation of benign and malignan nodules. |
