目的 比较实时剪切波弹性成像(shear-wave elastography,SWE)和超声造影(contrast-enhanced ultrasonography, CEUS)在乳腺BI-RADS 4类病变中的诊断价值。方法 对70例女性患者共72个乳腺病变进行SWE检查,测量最大弹性模量值Emax;进行CEUS检查,运用CEUS 5分法对病灶评分。以手术病理结果为诊断金标准,构建受试者工作特征(receiver operating characteristic,ROC)曲线,计算曲线下面积(AUC)并比较两种方法的诊断效能。结合BI-RADS亚分类比较诊断准确性。结果 通过绘制ROC曲线,Emax及CEUS评分对诊断BI-RADS 4类乳腺病变的AUC分别为0.840、0.754,两者无统计学差异(P >0.05)。对于4A类病变,SWE诊断准确性(93.9%)高于CEUS(75.7%)(P<0.05)。而对于BI-RADS 4B及4C类结节,SWE与CEUS的准确性无明显统计学差异(P >0.05)。结论 SWE和CEUS在乳腺BI-RADS 4类病变的良恶性鉴别诊断中有较高的诊断价值,且价值相当,与BI-RADS分类相结合可以优化检查方法。 |

Objective To compare the diagnostic performances of shear-wave elastography(SWE) and contrast-enhanced ultrasonography(CEUS) in differentiating the BI-RADS 4 breast lesions. Methods 70 patients with 72 lesions were examined with SWE and CEUS respectively. Maximum elasticity (Emax) and CEUS 5-point scores were assessed. The results were compared using the area under the receiver operating characteristic curve (AUC). To combine with BI-RADS, the accuracy of Emax and CEUS 5-point scores were compared. Results The AUC of Emax and CEUS 5-point scores were 0.840、0.754. No significant difference was noted. The accuracy of Emax(93.9%) was higher than that of CEUS 5-point scores(75.7%)in 4A lesions(P<0.05). There were no significant differences in accuracy among other categories. Conclusions Both SWE and CEUS were helpful for the differentiation the BI-RADS 4 breast lesions , and the diagnostic performance of SWE and CEUS was similar. |
