目的:研究超声引导下麦默通治疗乳腺导管内乳头状瘤(intraductal papilloma,IP)术后复发情况、分析复发的可能原因。方法:回顾性分析104例病理证实为IP患者的临床资料,所有患者均采用超声引导下麦默通旋切系统进行切除,应用超声密切随访,观察疗效、术后复发,进一步分析导致复发的可能原因。结果: 术后乳腺超声随访时间3-45个月(平均35个月),总共有14例超声检查显示原位置有结节形成,详细如下:随访至3个月原位复发1例,6个月原位复发3例,21个月时原位复发3例,33个月时原位复发3例,45个月时原位复发4例。统计分析示病灶复发与病灶的数量存在着统计学意义(P<0.05),与患者的年龄、病灶的大小、病灶的BI-RADS分级不存在统计学意义;病灶的复发时间与四个因素均无统计学意义。结论:麦默通微创旋切系统是一种治疗IP的可供选择的方法之一,但应该严格控制适应症条件,当术前怀疑非单发性的乳头状瘤时,慎用麦默通手术。 |

Postoperative recurrence and causes analysis of Mammotome biopsy under ultrasound control in the treatment of intraductal papilloma of the breast |

Objective:To investigate the postoperative recurrence and the possible causes of recurrence of Mammotome biopsy under ultrasound control in the treatment of intraductal papilloma of the breast。Methods:The clinical profiles of 104 patients with IP were retrospectively analyzed. All patients underwent Mammotome biopsy under ultrasound control resection and were followed up by ultrasonography to observe the curative effect postoperative recurrence,it is for further analysis of the possible causes of recurrence. Results :The total of 14 cases of ultrasound showed nodules in the original location in the follow-up time of 3-45 months (mean 35 months) with ultrasonography, as follows: Follow-up to 3 months in situ recurrence in 1 case, 6 months of the original 3 cases had recurrence, 2 cases had relapse in situ at 21 months, 3 cases had recurrence at 33 months, 4 cases had recurrence at 45 months. Statistical analysis showed that the number of recurrence and the number of lesions was statistically significant (P <0.05), and the patient"s age, lesion size, lesion BI-RADS grading is not statistically significant; The recurrence time and four factors have no statistical significance.Conclusion: The indications should be strictly controlled,though the Mammotome biopsy system is an alternative treatment of IP. It should be careful used when the preoperative suspected non-solitary papilloma. |
