【】 目的 评价超声定量非酒精性脂肪肝的诊断价值,并探讨肝脏脂肪含量与代谢综合征、大血管病变的相关性。方法 选取我院健康体检中心超声初步诊断的非酒精性脂肪肝患者约148例,通过超声定性诊断分为轻、中、重度脂肪浸润组,正常对照组约49例,应用超声定量方法估测各组的肝脏脂肪含量,并结合体重指数、血压、血液生化检查及颈动脉内膜-中层的厚度及斑块情况,比较各组间指标的差异性,进一步分析肝脏脂肪含量与各指标的相关性。结果 在校正了年龄及性别的影响后,正常对照组、轻度脂肪浸润组、中度脂肪浸润组和重度脂肪浸润组除收缩压、总胆固醇外,其他指标间差异有统计学意义(P<0.05)。肝脏脂肪含量与肥胖、糖尿病、脂代谢紊乱、高血压、颈动脉内-中膜病变及肝酶的改变呈显著相关性。结论 超声定量方法能较准确地估测肝脏脂肪含量,并与代谢综合征及颈动脉内-中膜病变密切相关,为临床定量诊断脂肪肝、随访和评估脂肪肝个体治疗效果提供了一种简易有效的辅助诊断工具。 |

【】 Objective TO evaluate the value of ultrasound in quantitative diagnosis of nonalcoholic fatty liver disease,and explore the correlation between liver fatty contents and metabolic syndrome、macro-vascular disease。Methods 148 patients from Physical Examination Center who were diagnosed NAFLD by ultrasound,were divided into light、medium、heavy fatty infiltration by qualitative diagnosis of ultrasound,49 were normal group。The liver fatty contents were estimated by ultrasound quantitative analysis in the object of study,and combined with BMI、BP、hematologic chemical and endocrine exams、carotid artery IMT and the clot were compared the difference in all groups,and analyzed the correlation between liver fatty contents and all the index above-mentioned。Results Adjusted by gender and age,beyond SBP、TC,other index were statistically significant (p<0.05)。liver fatty contents had significant correlation with obesity、DM、fatty metabolism、hypertension、carotid artery IMT and liver enzyme。Conclusion Ultrasonic quantitative diagnosis methods can be more accurate in estimating liver fatty contents。liver fatty contents is closely related with metabolic syndrome、carotid artery IMT。This method can provide a simple and effctive diagnostic tool in the diagnosis of fatty liver、follow-up and estimate the effectiveness in fatty liver for clinical。 |
