    目的 探讨超声引导经皮双极射频消融(radiofrequency ablation,RFA)治疗良性甲状腺结节的局部疗效。方法 超声引导下对56例良性甲状腺结节患者56枚结节行经皮双极射频消融治疗,采用常规超声及超声造影于术后1,3,6个月时随访观察结节的大小、血供及并发症发生情况。结果 所有患者均成功施行BRFA,术中2例出现声音嘶哑,暂停手术后声音恢复正常,考虑为局麻药一过性反应; 术后2例有穿刺部位红肿,未作特殊处理1周后复查红肿消失。BRFA术后6个月100%(56/56)结节体积缩小率≥50%,32.1%(18/56)结节体积缩小>90%。结论 双极RFA治疗良性甲状腺结节局部疗效显著,是有效和安全的,值得临床推广应用。

    Abstract:

    Objective To evaluate the local therapeutic efficacy of ultrasound (US)-guided bipolar radiofrequency ablation (BRFA) for benign thyroid nodules. Methods 56 patients with 56 benign thyroid nodules were treated with ultrasound-guided radiofrequency ablation. The local therapeutic efficacy were observed with 1, 3, 6 month after the treatment. Nodule volume, thyroid function and clinical symptoms of all the patients were compared before treatment and during follow-up, and the complications and thyroid function were assessed. Results The procedures were tolerated well in all the patients without causing any severe complications. Two patient experienced temporary nerve palsy, and recovered within 30 minutes after suspending the operation. Two patient bleed slightly after RFA,and the blood were disappeared after a week. At the 6-month follow-up, all (100%) the 56 nodules had volume reduction ratios (VRRs) of more than 50%, among which 18 (32.1%) had VRRs more than 90%. Conclusion Ultrasound-guided percutaneous BRFA for benign thyroid nodules seems to be an effective and safe method. It may be used widely in clinical practice.
