目的 应用射频超声血管定量技术检测乙肝患者的颈总动脉弹性变化,评价其血管功能。 方法 选取40例乙肝病毒(HBV)表面抗原阳性的肝病患者和34例年龄、性别匹配、无肝病病史的健康人,对以上两组研究对象分别进行肝脏声辐射力脉冲成像检查和颈总动脉射频超声检查,记录颈总动脉弹性参数和肝脏硬度参数,前者包括血管内中膜厚度(IMT)、脉搏波传导速度(PWV)、动脉管壁扩张性系数(DC)和顺应性系数(CC)、硬度指数、硬度指数,转折点压力P(T1),后者为剪切波传导速度ARFI,比较两组各项参数,并行统计学分析。 结果 乙肝患者组ARFI值明显高于对照组(2.07±0.67 m/s vs 1.03±0.10 m/s, P<0.001)。乙肝患者组颈总动脉的QIMT高于对照组(581.8±115.1 um vs 523.4±72.2 um, P<0.05)。PWV(7.52±1.71 m/s vs 7.83±1.63 m/s)、CC(0.90±0.37 /kPa vs 0.78±0.30 /kPa)、(5.19±2.48 vs 5.66±2.31)、DC、、P(T1)等弹性参数两组间未见显著性差异(P>0.05)。结论 乙肝患者的肝脏硬度和颈总动脉内中膜厚度高于健康对照组,PWV、CC、等功能性参数没有显著性差异。利用超声检测技术对乙肝患者颈总动脉的监测,将有益于对其血管功能和结构的关注。 |

Objective To explore changes in common carotid artery elasticity of HBV patients using radio frequency ultrasonography for evaluation of artery function. Methods 40 patients with Hepatitis B virus infection and 34 age- and sex-matched healthy volunteers underwent ARFI imaging and radio frequency ultrasonography. Data and parameters recorded were as follows: ARFI velocity, intima-media thickness(IMT), pulse wave velocity(PWV), distention coefficient(DC), compliance coefficient(CC), stiffness parameter , stiffness parameter and turning point T1(PT1). Results were compared between two groups of study subjects. Results HBV Patients had significantly higher ARFI(2.07±0.67 m/s vs 1.03±0.10 m/s, P<0.001) and IMT(581.8±115.1 um vs 523.4±72.2 um, P<0.05)than the control group. No significant results were found for PWV(7.52±1.71 m/s vs 7.83±1.63 m/s)、CC(0.90±0.37 /kPa vs 0.78±0.30 /kPa)、(5.19±2.48 vs 5.66±2.31),DC, or P(T1) between groups. Conclusion HBV patients have higher ARFI values and increased IMT with no significant changes in functional arterial parameters, indicating that monitoring IMT using radio frequency ultrasonography |
