    目的 探讨超声弹性成像变应率比值法(strain ratio, SR) 联合”萤火虫”成像技术对甲状腺良恶性结节鉴别诊断价值。方法 收集2015年1月-2016年12月在我院诊治甲状腺结节48例患者,共64个结节,术前行常规二维超声、超声弹性成像及“萤火虫”成像检查,与术后病理结果为对照。结果 64个结节中良性结节35个,恶性结节29个。萤火虫技术对恶性结节钙化显示率为75.86%,高于二维超声显示率62.07%,两者有统计学差异(p<0.05)。恶性结节SR值高于良性结节(p <0.01),当SR≧2. 92,诊断恶性结节的敏感性和特异性分别为79.3% 和71.4%。SR联合 “萤火虫”成像技术整体诊断恶性结节的敏感性及特异度均高于SR技术,与病理检查结果高度相关。结论 SR联合“萤火虫”成像技术对甲状腺良、恶性结节的诊断具有实用价值。

    Abstract:

    Objective To discuss the clinical value of strain ratio(SR)of ultrasound elastography(UE) combined with Micropure Imaging in differential diagnosis of benign and malignant thyroid nodules. Methods 48 patients with 64 thyroid nodules were enrolled in our hospital from February 2015 to June 2016. All patients were underwent traditional ultrasonography, UE and micropure imaging before operation, and compared with the results of pathological examination. Results Of 64 thyroid nodules, 35 lesions were benign, and 29 lesions were malignant. The detection rate of calcification in malignant nodules was 75.86% with micropure imaging, was higher than that (62.07%)of traditional ultrasonograph, which with significant different(p<0.05). SR of malignant nodules was higher than benign nodules(p <0.01), and when SR≧2. 92, the sensitivity and specificity for thyroid malignant lesions diagnosis were 79.3% and 71.4%, respectively. SR combined with micropure imaging had a higher sensitivity and specificity for malignant lesions diagnosis than that of SR technology, and highly correlated with pathological findings. Conclusion Strain ratio of ultrasound elastography combined with micropure imaging was an useful technolgoy in differential diagnosis of benign and malignant thyroid nodules.
