目的 探讨原发性高血压动态动脉硬化指数与肾实质剪切波速度的相关性。方法 选择75例原发性高血压患者和60例健康对照者,通过 24 h动态血压监测分析血压参数特点,计算动态动脉硬化指数(AASI)。声触诊组织定量分析技术测量肾皮质、髓质的剪切波速度(SWV),分析AASI与肾皮质、髓质剪切波速度及肾功能的相关性。结果 原发性高血压患者组和对照组动态动脉硬化指数值分别为 0.59 ± 0.18和 0.40±0. 13( P < 0. 01);原发性高血压患者组肾皮质与髓质的剪切波速度明显高于对照组(2.34±0.86 m/s vs 1.81±0.56m/s;1.97±0.84 m/s vs 1.68±0.58m/s,P < 0. 05)。AASI与肾皮质和髓质剪切波速度呈正相关 ( r = 0. 325、0. 268 , P <0. 05),AASI与BUN、Cr呈负相关( r =―0. 325 , ―0.387,P <0. 01)。结论 原发性高血压动态动脉硬化指数与肾实质剪切波速度、肾功能关系密切,AASI可以反映高血压肾损害。 |

Abstract: |
Objective Aim to evaluate the correlation of ambulatory arterial stiffness index (AASI) and the shear wave velocity (SWV) of renal parenchyma in primary hypertension. Methods 75 primary hypertension patients and 60 healthy people were selected. Twenty-four-hour ambulatory blood pressure monitoring (ABPM ) was performed and the blood pressure parameters were calculated. The shear wave velocity of renal cortex and medulla were measured using virtual touch tissue quantification(VTQ). Results AASI in primary hypertension group was higher than that in healthy control group ( 0.59±0.18 vs 0.40±0.13, P <0.01). The SWV of renal cortex and medulla of hypertension were higher than that of controls (P<0.05). AASI was correlated with the SWV of renal cortex and medulla positively( r = 0. 325、0.268 , P <0 . 05). AASI was correlated with BUN and Cr impositively ( r =―0.325 , ―0.387,P <0. 01). Conclusion AASI was closely related to the shear wave velocity of renal parenchyma in primary hypertension. |
