    摘要:

    目的 比较甲状腺影像报告及数据系统(TI-RADS)和弹性成像技术单独应用和联合应用对甲状腺结节良恶性的诊断效能。方法 回顾性分析了203例甲状腺患者的222个甲状腺结节。应用常规超声,TI-RADS和弹性成像技术对甲状腺结节的良恶性进行判断。以病理结果为金标准,分别比较三种方法单独使用的诊断效能。其次,用不同的联合方法,比较三种方法联合诊断的诊断效能。结果 TI-RADS诊断方法的敏感性92.4%,特异性85.4%,和有经验的操作者常规超声的诊断结果(敏感性90.8%,特异性81.6%)基本相同,弹性超声单独使用诊断效能稍低,敏感性72.3%,特异性87.4%。TI-RADS分级诊断联合弹性成像技术可以提高诊断效能。结论 在甲状腺结节良恶性的诊断上,TI-RADS方法与弹性成像技术联合诊断可以提高诊断效能。

    Abstract:

    Objective: To compare the diagnostic efficacy on benign and malignant thyroid nodules by TI-RADS and ultrasonic elastography alone or combined usage. Methods: Totally 203 cases of thyroid patients with 222 thyroid nodules were analyzed retrospectively. The benign and malignant thyroid nodules were evaluated by ultrasound, TI-RADS and elastography. The diagnostic efficacy of each of the three methods was compared according to the pathological results. Then the combined diagnostic performance of with the three methods were detected. Results: The mdiagnostic performance of TI-RADS(sensitivity 92.4%, specificity 85.4%) was almost the same as that of an experienced operator using ultrasound (sensitivity 90.8%,specificity 81.6%). The diagnostic performance of elastography was lower, the sensitivity was 72.3%, and the sensitivity was 87.4%. The TI-RADS combined elastography can improve diagnostic performance. Conclusions: In the diagnosis of benign and malignant thyroid nodules, TI-RADS method combined elastography technology can improve diagnostic efficiency.
