目的 利用超声造影(contrast-enhanced ultrasonography,CEUS)定量技术,探讨超声造影在兔急性肾功能衰竭肾皮质血流灌注变化的应用价值。方法 于16只新西兰白兔后腿多点肌注50%甘油(12-15ml/kg),建立急性肾功能衰竭模型,分别在注射前与注射后24h进行超声造影检查,采用Q-LAB软件对时间-强度曲线(TIC)进行分析,造影参数包括造影剂始增时间(AT),达峰时间(TTP),峰值强度变化(A),曲线上升支斜率(β)及曲线下面积(AUC)。结果 注射前肾皮质A和β值分别为(17.24±10.31dB)和(5.24±2.10dB/s),明显高于注射后24h的(7.32±4.81dB)和(1.62±1.20dB/s)(P<0.05)。注射前肾皮质TTP和AUC值分别为(2.48±1.81s)和(325.27±121.60dB/s),明显低于注射后24h的(6.10±3.41s)和(655.39±271.80dB/s)(P<0.05)。兔肾衰竭前、24h后AT、血肌酐、尿素氮、肾段动脉阻力指数(resistance index,RI)和收缩期峰值流速(peak systolic velocity,PSV)的差异无统计学意义(P>0.05)。结论 超声造影定量技术能较早并有效地反映兔急性肾功能衰竭皮质的血流灌注情况,比血肌酐、尿素氮生化指标更敏感,有助于对急性肾衰竭的早期诊断,具有一定的应用价值。 |

Abstract: |
Objective: To discuss the application of CEUS in quantitative evaluation renal cortical and medullary perfusion in acute renal failure in rabbits. Methods: Rabbit ARF models were established with intramuscular 50% glycerin (12-15 ml/kg) injection into 16 New Zealand rabbits" thighs and carried out CEUS examination before and 24h after injection and analyzed the perfusion time-intensity curve (TIC) by Q-LAB. The parameters including arrival time(AT), time to peak intensity(TTP), amplitude o f peak intensity(A), slope rate of TIC(β)and area under the curve(AUC). Results: the values of A and β of renal cortex before injection were (17.24±10.31dB)and (5.24±2.10dB /s), respectively, which was significantly higher than values after injection 24 hours later, i.e. (7.32±4.81dB)and(1.62±1.20dB /s)(P<0.05). The values of TTP and AUC of renal cortex before injection were (2.48±1.81s)and(325.27±121.60dB s),respectively, which was significantly lower than values after injection 24 hours later, i.e. (6.10±3.41s)and(655.39±271.80dB s)(P<0.05). Before renal failure in rabbits, the difference of AT, serum urea nitrogen (BUN)、serum creatinine (SCr)、resistance index(RI) and peak systolic velocity(PSV) after 24 hours were not significant. (P>0.05)Conclusion: the Contrast-enhanced ultrasonography quantitative technology can effectively reflect condition of the cortical and medullary perfusion in acute renal failure in rabbits. It is more sensitive than biochemical indicators of serum urea nitrogen (BUN), serum creatinine (SCr). It facilitates the early diagnosis of acute renal failure and has certain application value. |
