目的 探讨超微血流成像(SMI)技术在鉴别甲状腺肿块良恶性中的价值。方法 对97例甲状腺肿块患者的123个肿块于术前分别采用CDFI、PDFI及SMI技术探查肿块血流情况及血管分布特征,观察穿支血管的有无及数目,对比分析超声检查结果与病检结果。结果123个肿块中62个良性,61个恶性,CDFI、PDFI、SMI对良、恶性肿块检出的穿支血管数分别为0.10±0.47条及0.57±0.96 条、0.10±0.47条及0.72±1.07条、0.16±0.66条及1.61±1.74条。在123个肿块中,良、恶性肿块的穿支血管数差别有显著统计学意义(F=64.93,P=0.00),三种技术检出的穿支血管数良、恶性肿块间差异有显著统计学意义(F=10.50,P=0.00),SMI与CDFI、PDFI比较检出的穿支血管数差异有显著统计学意义(P=0.00),CDFI与PDFI比较差异无统计学意义(P>0.05);61个恶性肿块中SMI与CDFI、PDFI相比检出的穿支血管数差异有显著统计学意义(P=0.00),CDFI与PDFI相比差异无统计学意义(P>0.05),三种技术在62个良性肿块中检出的穿支血管数差异无统计学意义(F=0.296,P>0.05)。结论 穿支血管可作为甲状腺肿块良恶性的鉴别依据,SMI对穿支血管较CDFI、PDFI更敏感,可作为甲状腺肿块的良恶性鉴别的一种超声指标。 |

Objective To evaluate the superd mircovascular imaging(SMI) in detecting perfortor vessels for differentiating benign from malignant thyroid nodules. Methods Ninety seven patients with 123 thyroid nodules were detected by ultrasonography before operation and compared with the pathological examinations after operation. CDFI、PDFI and SMI were used to detect perfortor vessels of thyroid nodules respectively . Results There are 62 benign nodules and 61 malignant nodules in all thyroid nodules. The perfortor vessels of benign and malignant nodules detected by CDFI、PDFI and SMI were 0.10±0.4 and 0.57±0.96 、0.10±0.47 and 0.72±1.07、0.16±0.66 and 1.61±1.74 , respectively. In 123 thyroid nodules, there was a significant difference between the numbers of perfortor vessels in benign and malignant nodules(F=64.93,P=0.00). There was a significant difference among the numbers of perfortor vessels in all nodules and malignant nodules detected by SMI compared with CDFI、PDFI(P=0.00),but was no significant difference between the numbers of perfortor vessels in CDFI and PDFI(P>0.05). There was no significant difference among the numbers of perfortor vessels in benign nodules(P>0.05). Conclusions Perforators vessels can be used for distinguishing benign and malignant thyroid nodules. Compared with of CDFI and PDFI ,SMI is more sensitive to probe into the perforators vessels, may be applied in differentiation of benign and malignant thyroid nodules. |
