目的:应用三维超声成像技术对女性妊娠期及分娩后盆膈裂孔结构进行观察比较研究,探讨其盆底结构的变化。方法:选取我院在2013年1月至2016年5月间的妇产科妊娠时定期行产前检查并分娩的单胎初产妇80例。利用三维容积探头对受检者经会阴部进行检测,测得其盆膈裂孔在静息状态和Valsalva状态(深吸气后屏气)时其三维超声图,观察盆膈裂孔的形态和内部结构,并测量不同状态时盆膈裂孔的大小、面积等,比较其指标的差异与相关性。结果:通过观察三维超声对女性妊娠晚期及分娩后盆膈裂孔结构,结果可见,妊娠晚期女性的盆膈裂孔形态多呈“菱形”占58.75%, 而观察分娩后6周妇女的盆膈裂孔出现增大呈椭圆形或圆形,占51.25%;类菱形41.25%。进一步通过观测静息和张力状态下,妊娠晚期及分娩后的女性的盆膈裂孔的大小形态变化发现:与静息期比较,女性在分娩后其盆膈裂孔的的左右径、前后径及面积大小相较于其在妊娠晚期偏大;而与静息状态相比,妊娠晚期及分娩后的女性的盆膈裂孔的左右径、前后径及面积大小在张力期均增大。结论:利用会阴三维超声能够有效地动态观察女性盆膈裂孔的形态结构变化特点,女性在妊娠晚期与分娩后的盆膈裂孔显著增大,为产后妇女FPFD的早期预测提供可靠的影像学依据。 |

Abstract: |
Objective: To compare the anatomic changes of the pelvic floor in women during pregnancy and after childbirth using three-dimensional ultrasound imaging. Methods: Select 80 cases singletons primiparaes at the time of pregnancy and after childbirth in regular prenatal examination in obstetrics and gynecology of our hospital between January 2013 and June 2016. Perineum of three groups was examined using three-dimensional volume transducer at the resting state and state of Valsalva (breathless after deep inspiration). Observe its morphology and internal structure, and to measure the size and area of pelvic diaphragm hiatus of the three-dimensional ultrasonic tomograms, then compare their differences and correlation. Results: (1) Through observing the pelvic hiatus morphology of women in late pregnancy and delivery by the three-dimensional ultrasound, it is shown that the pelvic hiatus morphology was “rhombic” column crack-like structure at about 58.75% in late pregnancy, and “kind oval” at about 51.25% after delivery. Then observing the pelvic hiatus morphology of them in the resting and tension state, the shape variation and the size of the area was found to be increased in tension period then in the resting state. Conclusions:The variation of the pelvic floor structure of postpartum women can be observed dynamically by the perineum 3D ultrasound, and the late pregnancy women and after childbirth who will be significantly increases of pelvic diaphragm, which also proved reliable imaging references for clinic to predict of FPFD after delivery women. |
