| 【】目的：探讨高强度聚焦超声消融(海扶)治疗子宫切口瘢痕妊娠的疗效。方法：选取2012年12月至2015年12月我院确诊治疗的子宫切口瘢痕妊娠患者100例,依据治疗方法分为海扶组和电切组,每组50例,电切组给予常规甲氨蝶呤结合宫腔镜电切术治疗,海扶组在此基础上给予海扶术治疗,统计分析所有患者术出血量、手术、人绒毛膜促性腺激素(β-hCG)复常、月经恢复时间、并发症、1年内妊娠结局和术前术后血β-hCG水平情况。结果：海扶组术出血量、β-hCG复常、月经恢复时间、并发症发生率、复发率明显低于电切组,前者再次正常妊娠率明显高于后者,差异有统计学意义(P<0.05),但两组手术时间基本相同,差异无统计学意义(P>0.05)；海扶组和电切组术后血β-hCG水平较术前下降,但前者术后7d血β-hCG水平明显低于后者,差异有统计学意义(P<0.05)。结论：在甲氨蝶呤结合宫腔镜电切术中,联合海扶治疗子宫切口瘢痕妊娠疗效确切,可有效减少患者手术创伤及并发症,有利于改善患者1年内妊娠结局。 |

| 【S】objective:To discuss the curative effect of high intensity focused ultrasound ablation(HIFU) for the treatment in uterine incision scar pregnancy.Methods: 100 patients with uterine incision scar pregnancy were selected from December 2012 to December 2015 in our Hospital, according to the treatment method, all patients were divided into HIFU group and electric cut group, 50 cases in each group,electric cut group was given methotrexate combined hysteroscopic surgery treatment, HIFU group on the basis was given HIFU surgery treatment,statistical analysis of all patients with intraoperative blood loss, surgery, human chorionic gonadotropin(beta-hCG) often, menstrual recovery time, complication, 1 year pregnancy outcomes and preoperative,postoperative blood beta hCG levels.Results:The intraoperative blood loss, beta hCG often, menstrual recovery time, complications and recurrence rate of HIFU group were significantly lower than those of electric cut group,the again normal pregnancy rate of the former was significantly higher than that of the latter, the difference was statistically significant (P < 0.05), but the operation time of two groups were basically the same,here was no statistically significant difference (P > 0.05).Compared with preoperative,the postoperative blood beta hCG levels of HIFU group and electric cut group were decline,but the 7 d postoperative blood beta hCG level of the former was significantly lower than that of the latter, the difference was statistically significant (P < 0.05).Conclusions:In the methotrexate combine hysteroscopic surgery, combine HIFU has good curative effect in uterine incision scar pregnancy, which can effectively reduce the surgical trauma and complication of patients,it is helpful to improve the pregnancy outcome within 1 year. |
