    运用SPSS逐步判别分析方法建立有无冠状动脉粥样硬化性心脏病(以下简称冠心病)的分类函数方程,探讨动脉僵硬度定量分析(QAS)技术相关弹性参数诊断冠心病的临床价值。 方法 选取于我院心内科住院并行冠状动脉造影的疑似冠心病患者154例,按有无冠心病分为非冠心病组49例和冠心病组105例,两组均行颈动脉QAS检查,记录相关弹性参数;采用逐步判别分析方法建立分类函数模型。 结果 冠心病组PWV、CC、DC、α、β、R、H与非冠心病组比较,差异均有统计学意义(均P<0.05)。最终进入判别分析模型的指标为:顺应性系数(CC)、脉搏波传导速度(PWV)、反射波幅值(H)及反射波点斜率(R),所建立的Fisher判别方程为:Y= -7.985-0.612CC+0.298PWV+0.028H+5.058R,建立的Bayes判别方程为:Y(非冠心病)=-52.566+4.678CC+2.228PWV+0.629H+33.549R,Y(冠心病)=-63.509+3.793CC+2.659PWV+0.669H+40.869R。使用回顾性检验、交叉法检验及外部数据验证法检验判别方程效果的正确率分别为79%、75%及81%,误判率分别为21%、25%及19%。结论 CC、PWV、H、R这四个弹性参数与冠心病关系密切,利用逐步判别分析所建立的判别方程对判断有无冠心病有一定参考价值。

    Abstract:

    ABSTRACT Objectives Stepwise discrimination analysis method was adopted to establish function model of coronary heart disease(CHD) with SPSS software.And the clinical values of the relevant parameters by Quality Arterial Stiffness(QAS) were studied. Methods A total of 154 patients that doubtful coronary heart disease hospitailed in our cardiovascular department, undergoed coronary angiography and divided into non-CHD group(n=49) and CHD group(n=105).All the patients were checked by carotid QAS,and the relevan parameters were recorded.Stepwise discrimination analysis method was adopted to establish function model of CHD .Results The difference of PWV、CC、DC、α、β、R、H was statistically significant between non-CHD group and CHD group.At last, Pulse Wave Velocity (PWV),Compliance Coefficient(CC),the reflected wave amplitude(H)and slope(R)came into the model,and the Fisher discrimination function was Y=-7.985-0.612CC+0.298PWV+0.028H+5.058R,The Bayes discrimination function was Y(Non-CHD)=-52.566+4.678CC+2.228PWV+0.629H+33.549R,Y(CHD)=-63.509+3.793CC+2.659PWV+0.669H+40.869R. The accuracy rate and false rate by retrospective inspection,Jackknife method inspection and External data validation were respectively 79% and 21%, 75% and 25%,81% and 19%.Conclusions The parameters including PWV、 CC 、R and H were closely related to coronary heart disease,and the established function model had an important reference value in discriminating the occurrence of CHD.
