目的:探讨常规超声联合声触诊组织成像 定量(VTIQ)技术诊断Ⅱ型糖尿病周围神经病变(DPN)的价值。方法:回顾分析2015年12月-2017年1月DPN患者44例(DPN组)、健康志愿者30名(正常对照组),观察两组胫神经声像图特点,并测量比较横截面积(CSA)及VTIQ的剪切波速度(SWV)值。结果:DPN组受累神经肿大,神经束回声减低,“蜂窝状结构”模糊,神经外膜厚且不平整;CSA及SWV平均值较正常对照组高,两组差异有统计学意义(P<0.05)。结论:常规超声能较清晰显示受累神经结构及病变范围,VTIQ能反映受累神经的硬度,两者结合可以进一步提高诊断DPN的能力。 |

Purpose: To evaluate the value of conventional ultrasound combined virtual touch tissue imaging quantification(VTIQ)in typeⅡdiabetes with diabetic peripheral neuropathy. Method:Forty-four patients with diabetic peripheral neuropathy(DPN group) and thirty healthy volunteers (normal control group) were retrospectively analyzed from December 2015 to January 2017.The ultrasonographic features of tibial nerves were observed. Besides, the cross-sectional area (CSA) and shear wave velocity (SWV) were measured. Results: The tibial nerves of DPN group were wider than those of normal control group, the echo was decreased, “cellular structure” was fuzzy, and epineurium became thicken. The value of CSA and SWV were increased compared with normal control group, the difference was statistical significance(P<0.05). Conclusion: Conventional ultrasound (US) can show the nerve structure and scope of lesions of patients with DPN. It can provide morphological information. VTIQ can reflect the hardness of the affected nerve, and provide functional information. The combination methods can further improve the diagnostic ability of DPN. |
