目的 以18F- FDG PET-CT心肌葡萄糖代谢与13N-NH3 PET-CT心肌灌注显像为“金标准”,对比研究评价超声二维斑点追踪技术(ultrasound two - dimensional spot tracking imaging technology ,STI)检测ICM患者存活心肌的价值 方法 30例临床确诊为ICM的患者,均行完全血运重建治疗并在在完全血运重建前一周内完成常规超声心动图、STI及PET-CT检查 结果 30例患者共480个心肌节段,PET-CT心肌代谢显像见灌注正常的节段85个,灌注减低的节段395个,结合心肌代谢显像,共310个节段出现灌注-代谢不匹配,提示有存活心肌;85个节段出现灌注-代谢匹配,提示无存活心肌。STI分析表明,同一心肌节段,有存活心肌的节段与无存活心肌的节段其峰值RS存在显著性差异 结论 二维超声斑点追踪技术峰值RS可以很好地预测ICM患者的心肌活性,因此可以用于存活心肌的检测,并有望取代PET/CT在临床普及应用。 |

Determination of myocardial viability in ischemic patients:the value of two-dimensional ultrasound speckle tracking technology |

Abstract: |
Objective To investigate the value of two-dimensional ultrasound speckle tracking technology, putting 18F-FDG PET as the gold standard. Methods 30 cases with ischemic cardiomyopathy, underwent PET-CT imaging、regular UCG and STI. Results PET-CT imaging showed 395 segments with perfusion reduce, combined with myocardial metabolic imaging, a total of 310 segments perfusion - metabolic didn"t match, which means the survival myocardium; and 85 segments showed Perfusion-metabolic match, which suggested unviability. By analysis of the mate of STI, the peak RS of segments with viability was higher than that without, and the difference was significant, Conclusion The two-dimensional ultrasound speckle tracking technology peak RS is a good way to predict cardiac activity, and can be used for detecting survival myocardium, and hopeful to take of PET/CT in clinical application. |
