目的 探讨经胸超声心动图(Transthoracic Echocardiography,TTE)、CT及二者联合对缩窄性心包炎(constrictive pericarditis,CP)的诊断价值。 方法 回顾性分析我院及南充市中心医院2010年2月至2017年9月7年间74例临床疑似CP患者的TTE及CT影像学表现(其中经手术病理证实为CP患者54例)。分别由两位经验丰富的超声及CT医师分析TTE及CT表现并做出诊断。绘制TTE、CT及二者联合应用诊断CP的ROC曲线,计算曲线下面积,行Z检验。对已确诊为CP患者的影像学参数作χ2分析。 结果 TTE、CT及二者联合诊断CP的ROC曲线下面积分别为0.948、0.912、0.981,TTE及CT曲线下面积比较,无统计学差异(P >0.05)。二者联合与单用TTE或与单用CT曲线下面积比较,前者较高,差异均有统计学意义(P 均<0.05)。 结论 TTE与CT诊断CP的效能无差别,TTE可作为筛查、诊断的首选检查方法,CT可作为TTE诊断不足补充,并对手术方案制定提供重要信息;二者联合能对CP的诊断与治疗提供更加可靠的依据。 |

Comparative study of the diagnostic value using ultrasound, CT and their combination in constrictive pericarditis
ABSTRACT
Objective: To investigate the diagnostic value in constrictive pericarditis (CP)by transthoracic echocardiography (TTE ),CT and their combination . Methods: Seventy-four suspected CP patients by Clinical manifestation (including 54 patients with CP which confirmed by surgery and pathology ) in our hospital and Nanchong city center hospital from February 2010 to September 2017 were retrospectively analyzed . Two experienced TTE physicians and two experienced CT physicians analyzed the imaging findings of TTE 、CT respectively,and analyzed their combination together using Five-level diagnosis criterion.The ROC curves of CP were drawn by TTE, CT and their combination. The area under the ROC curve was calculated, and the area under the curve was tested.Using χ2 to analysis the imaging parameters of the patients diagnosed with CP.Results: According to the ROC curve, the area under the curve of TTE、CT and their combination are 0.948,0.912and 0.981, respectively.The area under the curve of TTE and CT has no statistical difference (P>0.05). The differience between their combination and the area under the TTE curve or the CT curve is statistically significant (P < 0.05).Conclusions: There is no difference in diagnostic efficiency between TTE and CT. TTE can be used as the first choice for screening and diagnosis. CT can be used as a deficiency in the diagnosis of TTE and provides important information for the formulation of surgical procedures,but their combination can provide more reliable basis for the diagnosis and treatment of CP.
