【摘 要】 目的 评价四种多普勒超声指标预测大鼠肺动脉压力的准确性。 方法 8周龄,雄性大鼠41只,分为对照组(Con组 :n=11);野百合碱组(MCT组:n=30),干预4周。采用脉冲多普勒测定肺动脉血流加速时间(PAAT)、肺动脉射血时间(PET)、三尖瓣返流峰流速(TR peak)。以有创肺动脉压力(PAP)为金标准,评估四种超声指标预测PAP的准确性。结果 PAAT、PAAT/PET、PAAT/HR及TR peak与有创PAP具有相关性(r=0.469、0.49、0.267、0.173, P<0.05)。ROC曲线显示PAAT/PET的诊断价值最高,PAAT/PET 采用0.381为截断值评估PAP的敏感度是0.818,特异度为0.90。结论 应用多普勒超声评估大鼠PAP时,PAAT/PET较TR peak更能准确预测大鼠PAP。 |

Comparison of distinct Doppler Ultrasonography Modalities in Evaluation of Pulmonary Arterial Pressure in Rats with Pulmonary Arterial Hypertension |

【Abstract】 Objective To evaluate the accuracy in estimating pulmonary arterial pressure (PAP) with four ultrasonic Doppler modalities in rats treated with monocrotaline (MCT). Methods The total of 41 male rats at the age of 8 weeks were divided into control group( Con group: n=11) and monocrotaline group(MCT group: n=30). Four weeks after treatment, The pulmonary arterial acceleration time (PAAT), pulmonary arterial ejection time (PET) and tricuspid regurgitation peak flow velocity (TR peak) were measured with Continuous-Wave Doppler. The invasive PAP was applied as gold standard to assess the correlation with such following ultrasonic parameters as PAAT, PAAT/PET, PAAT/HR (heart rate) and TR peak. Results The PAAT, PAAT/PET, PAAT/HR and TR peak were closely negatively-correlated with the
invasive mean pulmonary arterial pressure (PAP)(R=0.49, 0.469, 0.267, 0.173; p<0.05).
TR peak had the lowest relationship with the mean pulmonary artery pressure. ROC curve showed that PAAT/PET had the highest diagnostic value and its cut off value of 0.381 had 0.818 and 0.90 in sensitivity and specificity respectively on estimating pulmonary arterial hypertension . Conclusion The ratio of PAAT to PET was more reliable to assess pulmonary artery pressure in rats compared with TR peak. |
