目的 声弹性成像评估宫颈成熟度预测临产时间的价值。 方法 选取本院住院治疗的符合要求的孕晚期孕妇作为研究对象180例,定义为TOL≤1周组102例和TOL>1周组78例。记录两组孕妇的一般资料、宫颈长度、宫颈宽度、胎先露高度、羊膜腔嵌入宫颈内口深度、B/A值。利用单因素和多因素分析筛选预测临产的独立影响因素,并利用ROC分析各指标潜在的预测效能。结果 宫颈长度、胎先露高度和B/A值是预测临产的独立影响因素。胎先露高度和B/A值呈正相关(r=0.472,P<0.05)。宫颈长度预测临产的AUC为0.708,其敏感度为35.3%,特异性为100.0%,胎先露高度预测临产的AUC为0.824,其敏感度为73.5%,特异性为73.1%;B/A值预测临产的AUC为0.834,其敏感度为97.1%,特异性为56.4%;胎先露高度联合B/A值预测临产的AUC为0.891,其敏感度为67.6%,特异性为96.2%。结论 B/A值预测临产具有一定的价值,胎先露高度联合B/A值预测临产更加准确,可作为预测临产的一种新的指标。 |

Abstract: |
Objective To explore the prediction value of cervical maturity evaluated by acoustic Elastography. Methods One hundred and eighty pregnant women of the third trimester who meet the requirements were divided into two groups. They were defined as TOL ≤ 1 week group of 102 patients and TOL> 1 week group of 78 patients. The general data, cervical length, cervical width, fetal exposure level, depth of embedded amniotic cavity and B/A value of two groups were recorded. Univariate and multivariate analyzes were used to screen independent influencing factors for predicting labor.And use ROC to analyze the potential prediction efficiency of each index.Results Cervical length, fetal exposure level and B/A value were independent predictors of labor, the fetal exposure height and B/A values were positively correlated.The AUC for cervical length was 0.780 with a sensitivity of 35.3% and a specificity of 100.0%;the AUC for fetal exposure height predicted for labor was 0.824 with a sensitivity of 73.5% and a specificity of 73.1%;the AUC for B/A value to predicte labor was 0.834, and the sensitivity was 97.1% , the specificity was 56.4%;the AUC of fetus exposed height combined with B/Avalue was 0.891,and the sensitivity was 67.6%, the specificity was 96.2% .
ConclusionsB/A value has a certain value to predict labor,fetus exposed height combined with
B/A value is more accurate to predict labor,it can be used as a new indicator of predictive labor. |
