目的 探讨实时剪切波弹性成像技术(SWE)在腕管综合征(CTS)诊断中的应用价值。
方法 选取90例CTS患者及20例健康体健者,测得正中神经的DW、DF、CSAW、CSAF、WEmean、FEmean及FRatio值,比较两组以上各值之间的差异并绘制ROC曲线。结果 CTS组CSAW、DW、WEmean与对照组比较,差异有统计学意义(P<0.05),FEmean、DF 、CSAF 、FRatio与对照组比较,差异无统计学意义,P>0.05,CSAW、DW、WEmean诊断CTS的ROC曲线下面积分别为0.718、0.654、0.942,WEmean较CSAW 及DW诊断效能差异有统计学意义(P<0.05),并与CSAW呈线性正相关。结论 SWE成像可以提供正中神经平均弹性模量值,反应正中神经的硬度,为CTS诊断提供一种无创、简便的诊断方法。 |

Abstract: |
Objective To explore the value of real-time shear wave elastorgraphy(SWE) in the diagnosis of carpal tunnel syndrome(CTS).Methods 90 patients with CTS and twenty healthy volunteers were enrolled.the median nerves of DW, DF, CSAW, CSAF, WEmean, FEmean,and FRatio were obtained. The differences of parameters between two groups were compared,and the ROC curve was drawn.Results The differences of CSAW,DW,and WEmean were statistically significant between CTS group and control group(P<0.05). The differences of FEmean,DF,CSAF,and FRatio were no statistically significant(P> 0.05). On ROC curve,the areas under the ROC curve of CSAW, DW, and WEmean was 0.718, 0.654, and 0.942 respectively.WEmean has the best diagnostic efficiency compared to CSAW and DW, and was linearly positively correlated to CSAW.Conclusion SWE imaging can provide the mean elastic modulus of the median nerve,responding the hardness of the median nerve,which provide a non-invasive and simple diagnostic method for CTS diagnosis. |
