    摘要:

    目的 应用剪切波弹性成像测定皮肤病变杨氏模量,探讨其在鉴别皮肤良恶性病变中的应用价值。方法 选取经手术病理确诊的皮肤病变患者72例(72个皮肤病变),均行常规超声和剪切波弹性成像检查。以病理结果为金标准,绘制诊断皮肤恶性病变的受试者工作特征(ROC)曲线,计算曲线下面积并确定其截断值。结果 良、恶性病变的平均杨氏模量测值分别为(20.3±3.3)kpa、(38.3±9.2)kpa,两者比较差异有统计学意义(P<0.05)。杨氏模量平均值诊断皮肤恶性病变的ROC曲线下面积为0.914,以30.5 kpa作为剪切波弹性成像诊断皮肤恶性病变截断值,其诊断敏感性、特异性分别为84.85%、76.92%。结论 剪切波弹性成像有助于皮肤良恶性病变的鉴别诊断,有一定的临床价值。

    Abstract:

    ABSTRACT Objective To assess the value of quantitative parameters of shear wave elastography (SWE) in the differential diagnosis of benign and malignant cutaneous lesions by measuring the Young's modulus value. Methods A total of 72 patients underwent real-time SWE to measure the Young's modulus value.According to pathologic results,ROC curve was drawn to evaluate the best cut-off value in differentiating benign and malignant lesions.Results There were 39 benign lesions and 33 malignant lesions in the total 72 cutaneous lesions.The mean Young's modulus value of benign lesions was(20.3±3.3)kpa,as compared with(38.3±9.2)kpa in malignant lesions(P<0.05).On ROC curve,the area under the curve(AUC) of the mean Young's modulus value was 0.914.Taking 30.5 kpa as the threshold of mean Young's modulus value,the sensitivity was 84.85% and specificity was 76.92%.Conclusion The mean Young's modulus value of cutaneous lesions can quantitatively assess the lesions'elasticity.SWE is helpful in differentiating malignant and benign cutaneous lesions.
