    【】 目的 探讨早孕期胎儿严重心脏畸形的筛查与诊断价值。 方法 对于2015年2月至2018年2月行11~13+6周胎儿行颈项透明层(NT)检查时进行胎儿心脏检查,采用四腔心切面(4CH)和三血管气管切面(3VT)彩色多普勒血流显像观察胎儿心脏结构,随访并记录最终结果。结果 总共检查2021例胎儿,早孕期筛查出心脏严重畸形19例,漏诊4例,检出率达到了82.6%(19/23)。所有心脏异常病例在4CH切面和/或3VT切面有异常表现。 结论 经过严格训练的超声医师在胎儿NT检查的同时,采用4CH和3VT切面彩色多普勒筛查胎儿严重心脏畸形是一种行之有效的方法。

    Abstract:

    【】 Objective To evaluate the value of screening and diagnosis of fetal severe cardiac malformation in the first trimester. Methods ContinuousS pregnant women coming to our hospital from February 2015 to February 2018 for 11~13+6 weeks scanning were included in this study.Fetal heart was observed by four-chamber (4CH) view and three-vessel-trachea (3VT) view combined with color doppler flow imging (CDFI) in order to screen severe cardiac malformation. Follow up and the final result were recorded. Results A total of 2021 cases were included in the study. There were 19 cases with severe cardiac malformation were detected, 4 cases were missed, and the detection rate reached 82.6% (19/23). All cardiac abnormalities were abnormal in 4CH and/or 3VT views. Conclusions It is an effective method to screen fetal severe cardiac malformation by using 4CH and 3VT view and CDFI at the first trimester after the rigorous training of ultrasound physicians.
