目的:探讨三维斑点追踪成像(3D-STI)在静息状态下诊断非心肌梗死冠状动脉多支重度狭窄的价值。方法:以2018年1月~2018年12在我院行冠状动脉造影的100例疑诊冠心病患者为研究对象,根据冠状动脉造影的结果将患者分为多支重度狭窄组(40例)、单支重度狭窄组(30例)和对照组(30例)。所有患者均进行三维超声心动图检查,采用3D-STI技术测量左室整体纵向应变(3D GLS)、整体圆周应变(3D GCS)、整体面积应变(3D GAS)和整体径向应变(3D GRS)。采用ROC曲线分析3D-STE诊断冠状动脉多支重度狭窄的价值。结果:多支重度狭窄组与单支重度狭窄组的3D GLS、3D GCS、3D GAS、3D GRS均明显低于对照组(P<0.05),多支重度狭窄组的3D GLS、3D GAS均明显低于单支重度狭窄组(P<0.05);3D GLS的AUC为0.78,当截断值为-12%时,其敏感性较高为85.1%;3D GAS的AUC为0.77,当截断值为-21%时,其特异性较高为80.2%;3D GLS与3D GCS、3D GAS、3D GRS任一指标并联使用,其诊断冠脉多支重度狭窄的敏感性均较单一指标高。结论:3D-STE在静息状态下对非心肌梗死冠状动脉多支重度狭窄有一定的诊断价值,尤其3D GLS、3D GAS可作为冠状动脉多支重度狭窄较为有效的诊断指标。 |

【ABSTRACT】Objective: To investigate the value of three-dimensional speckle tracking imaging(3D-STI) in the diagnosis of severe multi-vessel coronary artery stenosis of non-myocardial infarction in resting state. Methods: 100 patients with suspected coronary heart disease who underwent coronary angiography which was selected as the research objects in our hospital from January 2018 to December 2018. Patients were divided into severe multi-vessel stenosis group (40 cases), severe single-vessel stenosis group (30 cases) and control group (30 cases) according to the results of coronary angiography. All patients were detected by three-dimensional echocardiography, and 3D-STI technology was used to measure the global longitudinal strain (3D GLS), global circumferential strain (3D GCS), global area strain (3D GAS) and global radial strain (3D GRS) of the left ventricle. ROC curve was used to analyze the value of 3D-STI in the diagnosis of severe multi-vessel coronary artery stenosis. Results:The 3D GLS, 3D GCS, 3D GAS, 3D GRS of severe multi-vessel stenosis group and severe single-vessel stenosis group was significantly lower than that of control group (P<0.05). The 3D GLS, 3D GAS of severe multi-vessel stenosis group was significantly lower than that of severe single-vessel stenosis group(P<0.05). The AUC of 3D GLS was 0.78, and the sensitivity was 85.1% when the cutoff value was -12%. The AUC of 3D GAS was 0.77, and the specificity was 80.2% when the cutoff value was -21%. When 3D GLS is used in parallel with any of the indicators of 3D GCS, 3D GAS and 3D GRS, the sensitivity of diagnosis of severe multi-vessel coronary artery stenosis was higher than that of single indicator. Conclusion:3D-STI had certain diagnostic value for severe multi-vessel coronary artery stenosis of non-myocardial infarction in resting state, especially 3D GLS and 3D GAS could be used as more effective diagnostic indicators for severe multi-vessel coronary artery stenosis. |
