目的 探讨肝血管瘤超声造影 “动脉相和/或门脉相结节状环形渐进向心性强化与延迟相高或等增强”特征表现对不典型肝海绵状血管瘤(CHL)的诊断价值,并分析非特征造影表现的肝海绵状血管瘤超声造影表现及原因。方法 回顾分析67个经手术获得病理为海绵状血管瘤的不典型病灶超声造影图像,以血管瘤特征表现诊断不典型CHL并于病理结果对比,并与其中54个同期行CT/MRI检查的CHL病灶放射学结果相比较,评价超声造影特征表现对不典型CHL诊断价值。结果 超声造影对不典型CHL诊断正确率高于二维超声结合CDFI对不典型CHL诊断正确率(P<0.01),且≤3 cm不典型CHL病灶与>3 cm不典型CHL病灶相比,>3 cm不典型CHL病灶更符合血管瘤CEUS表现典型的特征。超声造影在诊断不典型CHL正确率略高于CT/MRI,但两者差异不具有统计学意义(P>0.05)。CT/MRI和超声造影联合诊断对不典型CHL病灶诊断正确率显著高于单独使用CT/MRI和超声造影(P<0.01)。实际工作中,超声医师结合临床表现及生化有助于诊断超声造影呈恶性肿瘤造影模式的不典型CHL。结论 血管瘤CEUS表现典型的特征对不典型CHL有较高的诊断价值,其正确诊断率能与MRI/CT相媲美。超声结合临床生化及放射学检查更能提高不典型CHL的诊断。 |

The value of contrast-enhanced ultrasonography in the diagnosis of atypical hepatic cavernous hemangioma |

Objective To investigate the diagnostic value of hepatic hemangioma with contrast-enhanced "arterial phase and / or portal vein nodular ring progressive progressive cardiac enhancement and delayed phase high or equal enhancement" for atypical hepatic cavernous hemangioma (CHL), and analysis Ultrasonographic findings and causes of hepatic cavernous hemangioma with non-characteristic angiography. Methods Retrospective analysis of 67 cases of surgically obtained ultrasonographic images of atypical lesions with cavernous hemangioma. The diagnosis of atypical choroids was performed with hemangioma characteristics and compared with pathological results, and 54 of them were performed with CT/MRI. The radiological findings were compared to evaluate the diagnostic value of contrast-enhanced angiography for atypical CHL. Results The diagnostic accuracy of contrast-enhanced ultrasonography for atypical CHL was higher than that of two-dimensional ultrasound combined with CDFI for atypical CHL (P<0.01), and ≤3 cm atypical CHL lesions were associated with >3 cm atypical CHL lesions. >3 cm atypical CHL lesions are more consistent with the typical features of hemangio CEUS performance. The accuracy of contrast-enhanced ultrasound in the diagnosis of atypical CHL was slightly higher than that of CT/MRI, but the difference was not statistically significant (P>0.05). The diagnostic accuracy of CT/MRI and contrast-enhanced ultrasound in the diagnosis of atypical CHL lesions was significantly higher than that of CT/MRI and contrast-enhanced ultrasound alone (P<0.01). In practice, the combination of ultrasound performance and biochemistry is helpful for the diagnosis of atypical CHL with contrast-enhanced ultrasound imaging. Conclusions The typical characteristics of vascular aneurysm CEUS have a high diagnostic value for atypical CHL, and its correct diagnosis rate is comparable to MRI/CT. Ultrasound combined with clinical biochemistry and radiology can improve the diagnosis of atypical CHL. |
