Abstract:Abstract:Objective To explore the application value of transvaginal color doppler ultrasound and real time shear wave elastography(SWE)in diagnosis of benign and malignant cervical lesion.Methods 49 cervical carcinoma patients diagnosed in our hospital from January 2017 to April 2019 were employed as research targets,53 CIN patients and 60 benign cervical lesions patients were respective employed as CIN group and benign group by random number table method.All patients underwent ransvaginal color doppler ultrasound、SWE and cervical biopsy,using biopsy results as the gold standard,comparing the differences among benign cervical lesions、CIN、cervical carcinoma in elasticity,using ROC curve to assess diagnostic value of elasticity in diagnosis of CIN and cervical carcinoma,and analysing the relation between elasticity and the degree of cervical lesions by Spearman rank correlation.Result Maximum elasticity and mean elasticity in the cervical carcinoma group was respective higher than that in the CIN group and benign group(P<0.017),there was no statistical difference between the benign group and CIN group in maximum elasticity and mean elasticity(P>0.017);In different physiological periods,there was no statistical difference among cervical anterior muscular layer in the mode of transversal ultrasound modes,cervical anterior and posterior muscular layer in the mode of longitudinal ultrasound modes in mean elasticity(P>0.05);In the different mode,there was no statistical difference among proliferative phase、secretory phase and menopause in the mean elasticity(P>0.05);Both maximum elasticity and mean elasticity can obtain good diagnostic efficiency,their cut-off were 42.27 and 47.17;AUC were 0.902 and 0.925,there was no significant difference between maximum elasticity and mean elasticity in AUC(Z=0.532,P=0.601);There was a significant positive correlation between maximum elasticity(ρ=0.584,P=0.000)or mean elasticity(ρ=0.721,P=0.000)and the degree of cervical lesions. Conclusion Maximum elasticity and mean elasticity obtained by SWE examination can bring satisfying diagnostic efficiency in diagnosis of cervical cancer and precancerous lesions,and mean elasticity is independent to patients’menstrual cycles and ultrasonic section,perhaps elasticity can be the significant observed indicator in diagnosis of benign and malignant cervical disease.
