目的:应用二维斑点追踪技术(two-dimensional speckle tracking echocardiography,2D-STI)评估尿毒症患者左心室心肌分层应变,研究其对尿毒症患者心肌的受累程度进行临床评估时的具体作用以及应用价值。方法:通过临床和病理检查确诊为尿毒症的患者被选为病例组(60例);健康志愿者对照组(60例)。使用分层应变技术来测量左室心内膜下、中层和心外膜下层心肌的纵向应变峰值(LS)及圆周应变峰值(CS),并比较结果。结果:两组左室心肌LS与CS均为心内膜下层心肌峰值较高,且由内至外逐层递减,心尖段峰值较高,且由基底段至心尖段逐段递增,差异有统计学意义(P<0.05);病例组两腔、三腔及四腔心切面各层心肌LS、GLS及△GLS较对照组对应各层心肌均有所减低,差异有统计学意义(P<0.05);两组间基底段、中间段及心尖段CS、GCS及△GCS比较无明显变化,差异无统计学意义(P>0.05)。结论:尿毒症患者在射血分数降低之前就已存在左室心肌与收缩功能的损伤,全层心肌的收缩功能均有不同程度的降低,而其中,心内膜下心肌功能受损最为严重。 |

Objective:To evaluate the left ventricular myocardial stratification strain in uremic patients with chronic renal failure by two-dimensional speckle tracking technique, and to study the clinical value of two-dimensional STI in evaluating the degree of myocardial involvement in patients with chronic renal failure (uremic stage). Methods:Sixty uremic patients diagnosed clinically and pathologically were selected as case group and 60 healthy volunteers as control group. The longitudinal and circumferential strain peaks of the left ventricular myocardium in the subendocardial, middle and subepicardial layers were measured by STI technique, and the results were compared and analyzed.Results:The LS and CS of left ventricular myocardium in both groups decreased from Endo to Epi, and increased from basal to apical segment with statistical significance (P < 0.05); the LS, GLS and △GLS of each layer of two-chamber, three-chamber and four-chamber cardiac sections in the case group were lower than those in the control group (P < 0.05); there was no significant difference in CS, GCS and △GCS of basal, middle and apical segments between the two groups (P < 0.05). There was no significant difference (P > 0.05). Conclusions:Left ventricular myocardium and systolic function were damaged in patients with chronic renal failure before ejection fraction decreased, and myocardial full-thickness systolic function was impaired to varying degrees, of which subintimal myocardium was the most severely impaired. |
