目的:探讨高频超声和剪切波弹性成像结合临床指标评估II型糖尿病胫神经损伤的临床应用价值。方法:纳入糖尿病周围神经病变(DPN)组38例,无神经病变(NDPN)组34例,两组均测量身高、体重、血压, 进行空腹及餐后2h血糖,糖化血红蛋白( HbA1c),血清胰岛素样生长因子-1(IGF-1)、血浆同型半胱氨酸(Hcy)、神经电生理的检测;高频超声测量胫神经的前后径、左右径、横截面积,并进行弹性模量值测量。结果:DPN组的胫神经左右径、前后径、横截面积、弹性模量值及Hcy值均高于NDPN组,IGF-I低于NDPN组;以上四值与Hcy、HbA1c呈正相关,与IGF-1呈负相关;病程、横截面积、弹性模量值、HbA1c是NPD的独立危险因素,而IGF-1是保护因素,列线图结果显示评分达180分周围神经损伤的风险为70%。结论:四项独立危险因素和一项保护因素相结合对评估糖尿病胫神经损伤的发病风险有一定临床应用价值。 |
