目的 应用彩色多普勒高频超声和能量多普勒(PD)技术对银屑病患者甲损害及指伸肌腱改变进行评估,探究超声在关节病型银屑病甲损害中的临床应用价值。 方法 回顾性分析2016年6月-2019年6月于我院确诊为银屑病患者共86名,其中关节病型银屑病患者46例(PsA组),寻常型银屑病患者40例(Ps组)。将同期无甲损害的健康体检者30例为对照组。对所有患者进行体格检查,记录患者一般临床资料,高频超声测量指甲甲板、甲床、甲基质及指伸肌腱的厚度,应用Wortsman分类对指甲进行形态学分类,PD评估指甲局部炎症情况。结果Ps组患者银屑病严重程度大于PsA组,而PsA组的炎症指数高于Ps组患者。两组患者的甲板、甲床及甲基质均增厚,且指伸肌腱厚度大于对照组。Ps组患者Wortsman分类以I型多见,而PsA组以II型多见。PD技术显示两组患者甲床和甲基质中的PD信号均明显增强,且两组指甲远端指节关节肌腱处PD信号均明显增强。结论 彩色多普勒高频超声能够评估银屑病患者甲结构和形态学改变,可反映甲床局部炎症情况,且对关节病型银屑病的发生发展具有重要评估价值。 |

Objective To evaluate the nail damage and extensor tendon changes in patients with psoriasis by color Doppler high frequency ultrasound and energy Doppler ultrasound, and to explore the clinical value of ultrasound in the diagnosis of nail damage in patients with arthritic psoriasis. Methods A total of 86 patients with psoriasis diagnosed in our hospital from June 2016 to June 2019 were analyzed retrospectively, including 46 patients with arthropathy psoriasis (PsA group) and 40 patients with psoriasis vulgaris (Ps group). 30 healthy subjects without nail damage in the same period were selected as the control group. All patients were examined by physical examination, and the general clinical data were recorded. The thickness of nail deck, nail bed, nail matrix and extensor tendon were measured by high frequency ultrasound. Wortsman classification was used to classify fingernails. PD was used to evaluate the local inflammation of fingernails. Results The severity of psoriasis in Ps group was higher than that in PsA group, and the inflammatory index in PsA group was higher than that in Ps group. The deck, nail bed and nail matrix of the two groups were thickened, and the thickness of finger extensor tendon was larger than that of the control group. Type I was the most common Wortsman classification in Ps group, while type II was more common in PsA group. PD showed that the PD signal in nail bed and nail matrix was significantly enhanced in both groups, and the PD signal in the tendon of distal fingernail joint was significantly enhanced in both groups. Conclusion Color Doppler high frequency ultrasound can evaluate the structure and morphology of nail in patients with psoriasis, reflect the local inflammation of nail bed, and has important value in the occurrence and development of arthropathy psoriasis. |
