目的 探讨极速脉搏波技术定量评价接受维持性血液透析(maintenance hemodialysis,MHD)治疗继发甲状旁腺功能亢进(secondary hyperparathyroidism,SHPT)的慢性肾功能衰竭患者颈动脉血管壁僵硬度情况。方法 选取30例继发甲状旁腺功能亢进的慢性肾功能衰竭并接受维持性血液透析治疗6-12个月的患者(MHD组)和34名健康志愿者(对照组),采用超声极速脉搏波(ultrafast pulse wave velocity,ufPWV)技术测量颈动脉收缩起始时脉搏波速度(PWV-BS)及收缩结束时脉搏波速度(PWV-ES),得到颈动脉血管壁僵硬度数据。 结果 与对照组比较,MHD组 PWV-BS值和PWV-ES值均明显升高,差异有统计学意义(P<0.05),且参数PWV-ES较PWV-BS升高更为显著。 结论 接受维持性血液透析治疗后继发甲状旁腺功能亢进的慢性肾功能衰竭患者颈动脉血管壁僵硬度增加,参数PWV-ES较PWV-BS能更敏感的反映出血管壁僵硬度变化。 |

Objective Ultrafast pulse wave velocity technique was used to evaluate the stiffness of carotid wall in renal failure with secondary hyperparathyroidism patients treated with maintenance hemodialysis.Methods A total of 30 patients with secondary hyperparathyroidism in chronic renal failure and receiving maintenance hemodialysis for 6-12 months were enrolled.The age and sex mached healthy cases were selected as control group(n=34). After conventional carotid ultrasound examination, ultrafast pulse wave velocity (ufPWV) was used to measure the pulse wave velocity of the begining-systole (PWV-BS) and the pulse wave velocity of the end-systole (PWV-ES), obtaining the stiffness of the carotid. Results Compared with the control group, PWV-BS and PWV-ES of MHD group were significantly increased, and the difference was statistically significant (P < 0.05). Conclusion Patients with secondary hyperparathyroidism in chronic renal failure undergoing maintenance hemodialysis, which is increased the stiffness of the carotid. The parameter PWV-ES is more sensitive than PWV-BS to reflect the change of vascular wall stiffness. |
