目的 运用声触诊组织定量技术探讨桥本氏甲状腺炎(Hashimoto's thyroiditis,HT)在不同临床进程中的组织弹性变化。
方法 回顾性分析153例经临床确诊的HT患者,所有患者均经过实验室血清学检查,正常对照组50例。根据HT的二维超声图像特征分为局灶型(I类)、类结节型(II类)、弥漫回声减低型(III类)、弥漫回声增强型(IV类),分别对应于HT的临床初期、进展期、活动期、恢复期;运用声触诊组织定量技术(virtual touch tissue quantification,VTQ)分析不同时期的HT的剪切波速度(shear wave velocity,SWV)变化。
结果 HT超声表现局灶型(I类)35例,类结节型(II类)59例,弥漫回声减低型(III类)27例,弥漫回声增强型(IV类)32例;对照组SWV值与HT组比较差异有统计学意义(P<0.05);HT组SWV值不同组别间除II、III类间差异无统计学意义,其余组别间比较差异有统计学意义(P<0.05);随HT临床进程的发展SWV值由I类逐渐升高至II、III类达峰值,至IV类逐渐下降。 |

Objective To explore elastic change in thyroid tissue by using virtual touch tissue quantification(VTQ) in the different clinical course of Hashimoto's thyroiditis(HT). Methods 153 patients with HT who were clinically and serological diagnosed and were retrospectively analyzed. 50 cases were in normal control group According to the characteristics of HT two-dimensional ultrasound images, the patients were divided into focal type (type I),nodular type (type II), diffuse echo reduction type (type III), and diffuse echo enhancement type (type IV) corresponding to early period, advanced period, active period and recovery period in the clinical course of HT. The change of the shear wave velocity(SWV) in thyroid tissue by VTQ in the process of HT was measured. Results The ultrasonic image types of HT included focal type(type I) in 35 cases, nodular type(type II) in 59 cases, diffuse echo reduction(type III) in 27 cases and diffuse echo enhancement(type IV) in32 cases. The difference about SWV is statistically significant between normal control group and HT(P<0.05).The differences about SWV are statistically significant between the different type in HT except type II and III(P<0.05). The SWV in HT was rising higher gradually from type I to type II and III, and descending gradually in the process of the type IV. Conclusion there was a significant change of the tissue elastic in the process of HT. The hardness of the Thyroid tissue was ascending gradually when the inflammation in HT was aggravated and was descending when the inflammation faded. |
