Abstract: |
Objective To explore the clinical value of high frequency ultrasound diagnosis and guided hydraulic release and Super-mini director release in the treatment of trigger finger.Methods Using Toshiba Aplio 500 color doppler ultrasound diagnostic instrument, two-dimensional image examination was performed on 26 patients with trigger finger, respectively recording the thickness of the A1 trochlea and the narrow part of tendon in the affected finger, and the same part of the contralateral finger.Different treatment methods are adopted according to clinical classification [1] . Patients with type I were treated with ultrasound-guided hydraulic release. Patients with type II were treated with hydraulic release under ultrasound guidance first, if the effect was not good,then A1 pulley release, or ultrasound –guided A1 trochlea release using Super-mini director directly. All Patients with type Ⅲ were treated with ultrasound –guided A1 trochlea release using Super-mini director.There were 4 fingers with Type I,13fingers with Type II and 13 fingers with Type III.Among them,there were 6 fingers with hydraulic release and 24 fingers with Super-mini director release .All patients were regularly reviewed and followed up for 6 months to evaluate the efficacy.
Results 1.Ultrasonic examination results: the thickness of A1 pulley of the affected finger was thicker than that of the contralateral healthy finger, and the difference was statistically significant (P < 0.05). Among 26 patiets, 4 cases were associated with tendon sheath effusion, and 1 case was associated with peri-tendinous cyst .2. Teatment results: Cure: 26 fingers; Improvement: 4 fingers. The cure rate was 86.67%, the effective rate was 13.33%, and the total effective rate was 100%.
Conclusion High-frequency ultrasound can accurately display the stenosis site and degree of the flexor tendon of trigger finger, and real-time dynamic guidance of hydraulic release or Super-mini director cutting is accurate, safe, minimally invasive and effective, which is worthy of clinical application. |
