目的:探讨超声造影引导细针穿刺对甲状腺乳头状癌(PTC)颈侧区转移性淋巴结的诊断价值。方法:回顾性分析2017年9月至2019年9月武汉科技大学附属孝感医院行术前颈侧区淋巴结细针穿刺并接受手术治疗的128例PTC患者影像病理资料,随机分为两组,对照组64例患者113个可疑淋巴结在常规超声引导下行细针穿刺(US-FNAC),实验组64例患者118个可疑淋巴结在超声造影引导下行细针穿刺(CEUS-FNAC),比较两组方法对颈侧区转移性淋巴结的诊断效能。并进一步分析US-FNA、CEUS-FNA对不同大小淋巴结的诊断效能。结果:US-FNAC诊断颈侧区转移性淋巴结的敏感性、特异性、准确率分别为72.3%、79.2%、75.2%;CEUS-FNAC诊断颈侧区转移性淋巴结的敏感性、特异性、准确率分别为90.2%、93.5%、91.5%,均高于US-FNAC,差异具有统计学意义(P值均<0.05)。当淋巴结最大径£10mm时,CEUS-FNA诊断转移性淋巴结的敏感性、准确率均高于US-FNAC,差异具有统计学意义(P<0.05),但特异性之间的差异无明显统计学意义(P=0.181);当最大径>10mm时,CEUS-FNA与US-FNAC的诊断效能差异无明显统计学意义(P值均>0.05)。结论:CEUS-FNA可提高PTC颈侧区淋巴结转移的诊断效能,具有重要的临床价值,可以有效指导手术治疗。 |

Objective To explore the value of contrast-enhanced ultrasound guide fine-needle aspiration cytology (CEUS-FNAC) in the diagnosis of lateral cervical lymph nodes (LCLNs) metastasis in papillary thyroid carcinoma (PTC). Methods Retrospective analyzed 128 PTC patients who accepted preoperative FNAC and thyroidectomy in Xiaogan Hospital affiliated Wuhan science and technology university during Sep. 2017 to Sep. 2019 were randomly divided into two groups.113 suspicious LCLNs of 64 patients in control group underwent conventional ultrasound guide fine-needle aspiration cytology (US-FNAC), 118 suspicious LCLNs of the other 64 patients in experimental group underwent CEUS-FNAC, the sensitivity, specificity and accuracy of US-FNAC and CEUS-FNAC in the diagnosis of LCLNs were compared. The diagnosis value of different size (£10mm or >10mm) of LCLNs by US-FNAC and CEUS-FNAC were analyzed. Result The sensitivity, specificity and accuracy of CEUS-FNAC in the diagnosis of LCLNs (90.2%、93.5%、91.5%) were significantly higher than those of US-FNAC (72.3%、79.2%、75.2%) (all P < 0.05). When the size of LCLNs £10mm, the sensitivity and accuracy of CEUS-FNA were higher than US-FNA (P < 0.05), but there was no significant difference of specificity (P = 0.181) ; when the size of LCLNs >10mm, there were no significant difference in the sensitivity, specificity and accuracy of the two methods (all P > 0.05). Conclusion CEUS-FNAC would favorable to improve the diagnosis accuracy of lateral metastatic lymph nodes, providing effective information for surgical treatment. |
