目的 探讨高分辨率血流(HD-FLOW)联合时间一空间复合成像(STIC)技术在帆状胎盘诊断的临床价值。方法 回顾分析超声诊断为帆状胎盘56例HD-FLOW及STIC血流图,选取同期正常胎儿为对照组,分析两组脐动脉血流频谱参数:S/D比值,搏动指数(PI)及阻力指数(RI),根据产后情况,判断彩色血流联合STIC技术在帆状胎盘的准确性。结果 2例引产后证实,5例产后证实为球拍状胎盘,VCI组胎儿及对照组胎儿脐动脉血流频谱指标比较,VCI组各项参数略高于对照组(P>0.05);51例VCI中有45例均获得了脐带胎盘插入点脐血管立体直观图,HD-Flow联合STIC诊断符合率高于二维联合HD-Flow技术。结论 HD-Flow联合STIC技术可用于辅助诊断VCI,具有重要的临床价值。 |

Objective: To observe the clinical value of HD-Flow combined with spatio-temporal image correlation (STIC) in the diagnosis of velamentous cord insertion (VCI). Methods: A total of 56 cases of VCI diagnosed using HD-Flow and STIC were selected as VCI group, and healthy fetuses were selected as normal control group. Umbilical artery blood flow spectrum parameters were measured in two groups including S/D ratio, pulsatility index(PI) and resistance index(RI). According to postnatal results, compare the accuracy and feasibility of HD-Flow combined STIC technology in diagnosis of VCI. Results: There were 2 cases were confirmed after induction, 5 cases were confirmed as racket-shaped placenta after delivery. The parameters of the VCI group were slightly higher than the control group, but there was no statistical significant (P> 0.05). In 45 of 51 cases, STIC images of the umbilical cord placental insertion point were obtained, and this technique provided a stereoscopic and visual image for the diagnosis of VCI. The diagnosis coincidence rate of HD-Flow combined STIC is higher than two-dimensional combined HD-Flow. Conclusion: Therefore, HD-Flow combined STIC technology can be used to assist the diagnosis of fetal VCI, and this technique has important clinical value. |
