目的 通过彩色多普勒超声研究早产儿在出生后3天内大脑中动脉(Middle cerebral artery,MCA)血流动力学变化与早产儿预后的相关性。方法 通过彩色多普勒超声于出生后3天内检测早产儿大脑中动脉血流参数收缩期血流峰值流速( Peak systole velocity,PSV),舒张末期血流速度(End diastolic velocity,EDV),搏动指数(Pulsatility index ,PI)及阻力指数(Resistance index,RI);并对早产儿的预后进行0~6岁小儿神经心理发育评估随访,根据出生后6个月进行的0~6岁小儿神经心理发育评估结果分成两组:发育商<85分为预后不良组、发育商≥85分为预后良好组。结果 出生后3天内早产预后不良组早产儿MCA-PSV、MCA-EDV低于预后良好组,MCA-PI及MCA-RI预后不良组高于预后良好组早产儿,两两比较差异有统计学意义(P<0.05)。以MCA-PSV、MCA-EDV高于或低于正常对照两个标准差为异常,MCA-RI≤0.5或MCA-RI≥0.75为血流指标异常,通过MCA-PSV、MCA-EDV及MCA-RI预测早产儿预后不良的敏感性分别为3.9%、9.8%、52.9%;特异性分别为98.6%、97.2%和72.2%。结论 应用彩色多普勒超声测定早产儿生后3天内大脑中动脉血流参数对预测早产儿有一定的临床价值,为临床早期诊断治疗提供有价值的依据,改善早产儿预后,减少并发症。 |

objective To investigate the correlation between hemodynamic changes in middle cerebral artery (MCA) and prognosis of premature infants within 3 days after birth.Methods Middle cerebral artery systolic peak flow velocity(PSV),end diastolic velocity(EDV),pulsatility index (PI)and resistance index(RI)were detected by color doppler flow image in 3 days after birth.The prognosis of premature infants was followed up and observed. According to the neuropsychological development assessment results of children aged 0 to 6 months after birth,they were divided into two groups:the developmental quotient<85 was divided into the poor prognosis group, and the developmental quotient ≥85 was divided into the good prognosis group.Results MCA-PSV and MCA-EDV of preterm infants in the group with poor prognosis within 3 days after birth were significantly lower than those in the group with good prognosis,and MCA-PI and MCA-RI group with poor prognosis were higher than those in the group with good prognosis,with statistically significant differences(P<0.05).The sensitivity of MCA-PSV,MCA-EDV and MCA-RI to predict poor prognosis of premature infants was 3.9%,9.8% and 52.9%,respectively.The specificity was 98.6%,97.2% and 72.2%,respectively. Conclusion The application of color doppler ultrasound to determine the blood flow parameters of middle cerebral artery in 3 days after birth of premature infants has certain clinical value for predicting premature infants,providing valuable basis for early clinical intervention. |
