目的:探讨超声在老年类风湿性关节炎膝关节病变的诊断及疗效评估中的应用。方法:选择2018年6月~2019年6月间医院收治的以膝关节疼痛为主诉的65例老年类风湿性关节炎患者为研究组,另选35例同期体检正常老年人为对照组。采用超声观察并比较两组受检者膝关节内彩色血流分级、髌上囊积液分级、滑膜厚度分级等情况,研究组患者接受3个月的标准化治疗,用超声评估研究组患者治疗后的疗效。结果:65例研究组患者的130个膝关节中,超声诊断出108个发生病变,诊出率为83.08%(108/130)。研究组患者的膝关节内彩色血流分级、髌上囊积液分级、滑膜厚度分级等超声诊断情况均与对照组有明显差异(P<0.05)。研究组患者的髌上囊液体厚度、滑膜厚度与对照组比较,差异具有统计学意义(P<0.05),而髁间软骨厚度比较,差异无统计学意义(P>0.05)。治疗后,研究组患者的髌上囊液体厚度、滑膜厚度均较治疗前明显变薄,滑膜内彩色血流信号减少(P<0.05),而髁间软骨厚度无明显变化(P>0.05)。65例研究组患者治疗后,45例自觉临床疼痛症状明显改善,13例有所缓解,7例无明显变化;停药后复发3例,经再次治疗后得到控制。结论:超声可早期诊断出老年RA患者的膝关节病变,并可用于评估老年RA患者膝关节病变的疗效,值得临床推广。 |

Objective:To explore the application of ultrasound in the diagnosis and curative effect evaluation of knee joint diseases in elderly rheumatoid arthritis.Methods:From June 2018 to June 2019, 65 elderly rheumatoid arthritis patients with knee pain as the main complaint in the hospital were selected as the study group, and another 35 normal elderly people were selected as the control group. Ultrasound was used to observe and compare the classification of color blood flow in the knee joint, the classification of hydrops in the suprapatellar capsule and the classification of synovium thickness between the two groups. The patients in the study group received standardized treatment for 3 months, and the therapeutic effect of the patients in the study group was evaluated by ultrasound.Results:Among the 130 knee joints of 65 patients in the study group, 108 lesions were diagnosed by ultrasound, and the diagnostic rate was 83.08% (108/130). There were significant differences between the study group and the control group (P<0.05). There was significant difference in the thickness of suprapatellar capsule and synovium between the study group and the control group (P<0.05), but there was no significant difference in the thickness of intercondylar cartilage (P>0.05). After treatment, the epithelium fluid thickness and synovial membrane thickness of the study group patients were significantly thinner than before treatment, and the color blood flow signal was reduced (P<0.05), while the intersacral cartilage thickness was not significantly changed (P>0.05).After treatment in 65 patients in the study group, 45 patients experienced a marked improvement in their clinical pain symptoms, 13 patients were relieved, and 7 patients had no significant changes. 3 patients relapsed after stopping treatment and were controlled after retreatment.Conclusions:Ultrasound can diagnose knee joint lesions in elderly RA patients early, and can be used to evaluate the efficacy of knee joint lesions in elderly RA patients. It is worthy of clinical promotion. |
