目的 分析超声引导下富血小板血浆联合药物注射治疗肩袖损伤的临床效果。方法 选取2018年12月—2020年4月期间,我院收治的肩袖损伤患者100例作为研究对象。根据随机数字表原则,将患者分为对照组和观察组,每组各50例。对照组患者给予超声引导下药物注射治疗,观察组患者给予超声引导下富血小板血浆联合药物注射治疗。分别于治疗前、治疗后及随访1个月时,利用视觉模拟(VAS)评分评价两组患者患肩疼痛程度,对比两组患者主动前屈关节活动度(AFROM)、主动外展关节活动度(AAROM),利用加州大学肩关节评分系统(UCLA)评价综合治疗效果,同时观察两组患者治疗期间的不良反应。结果 治疗后及随访一个月时,两组患者VAS评分均低于治疗前,且观察组患者VAS评分低于对照组,差异均具有统计学意义(P<0.05)。治疗后及随访一个月时,两组患者AFROM角度、AAROM角度均高于治疗前,且观察组患者上述指标高于对照组,差异均具有统计学意义(P<0.05)。治疗后及随访一个月时,两组患者UCLA评级均优于治疗前,且观察组患者UCLA评级优于对照组,差异均具有统计学意义(P<0.05)。两组患者治疗期间均未发生严重不良反应。结论 超声引导下富血小板血浆联合药物注射治疗肩袖损伤,具有较好的临床效果,可明显减轻患者疼痛,改善患者肩关节功能,且具有较好的安全性。 |

Objective To analyze the clinical effect of ultrasound-guided platelet-rich plasma combined with drug injection in the treatment of rotator cuff injury. Methods From December 2018 to April 2020, 100 patients with rotator cuff injuries admitted to our hospital were selected as the research object. According to the principle of random number table, the patients were divided into control group and observation group, 50 cases in each group. Patients in the control group were given ultrasound-guided drug injection therapy, and patients in the observation group were given ultrasound-guided platelet-rich plasma combined with drug injection therapy. Before treatment, after treatment and 1 month follow-up, visual analog (VAS) score was used to evaluate the degree of shoulder pain in the two groups. The active flexion range of motion (AFROM) and active abduction range of motion (AAROM) were compared between the two groups. The University of California at Los Angeles Scoring System (UCLA) was used to evaluate the effect of comprehensive treatment. At the same time, the adverse reactions were observed during treatment of two groups. Results After treatment and at one-month follow-up, the VAS scores of the two groups of patients were lower than before treatment, and the VAS scores of the observation group were lower than the control group, the differences were statistically significant (P <0.05). After treatment and during a month of follow-up, the AFROM angle and AAROM angle of the two groups of patients were higher than before treatment, and the above indexes of the observation group were higher than the control group, the differences were statistically significant (P <0.05). After treatment and at a month of follow-up, the UCLA ratings of the two groups of patients were better than before treatment, and the UCLA ratings of the observation group were better than the control group, the difference was statistically significant (P <0.05). No serious adverse reactions occurred during the treatment in both groups. Conclusion Ultrasound-guided platelet-rich plasma combined with drug injection for the treatment of rotator cuff injury has a good clinical effect, can significantly reduce patients "pain, improve patients" shoulder joint function, and has good safety. |
