目的 探讨脂肪肝患者腹部超声参数与血脂水平的相关性。方法 选取2018年1月至2019年10月期间就诊于我院的150例脂肪肝患者作为研究对象,按照血脂检测结果分为血脂异常组(n=116)和血脂正常组(n=34)。以各组患者脂肪肝超声参数为自变量,应用单因素分析和多因素Logistic回归分析来确立超声参数-血脂诊断模型,并通过模型计算得出的参数评分,分别与各血脂指标水平进行相关性分析。结果 经单因素及多因素分析发现,肝界增大角度圆钝、肝脏后方回声衰减、肝内血管变细-挤压及肝内血流色彩暗淡是脂肪肝患者合并血脂异常的敏感超声参数是与血脂异常密切相关的超声参数(P<0.05);脂肪肝患者超声参数-血脂诊断模型为:Logit(参数)=1.615X1 +1.933X2 +1.526X3 +1.335X4 -2.939;ROC曲线下面积AUC为0.831(95%CI:0.729-0.934),当超声参数以0.6分为分界值时,诊断模型敏感度87%,特异度68.4%;超声评分分值与TG、TC、LDL-C水平之间具有显著正相关性(r>0,P<0.05),与HDL-C水平之间具有显著负相关性(r<0,P<0.05)。结论 本研究构建的超声参数-血脂诊断模型能够较好的将脂肪肝患者超声参数与血脂水平结合,增强了二者之间的相关性,有利于提升脂肪肝人群的早期筛查和诊断。 |

Correlation analysis of abdominal ultrasound parameters and blood lipid in patients with fatty liver |

Abstract: |
Objective To investigate the correlation between abdominal ultrasound parameters and lipid levels in patients with fatty liver. Methods A total of 150 patients with fatty liver who were admitted to our hospital from January 2018 to October 2019 were selected as the study subjects.According to the results of lipid test, they were divided into the dyslipidemia group (n=116) and the normal lipid group (n=34).The ultrasonic parameters of fatty liver in each group were taken as independent variables, and the ultrasonic parameters-blood lipid diagnosis model was established by univariate analysis and multivariate Logistic regression analysis. The parameter scores obtained by the model were calculated and correlated with the levels of various blood lipid indexes.Results Through univariate and multivariate analysis, it was found that the sensitive ultrasound parameters of patients with fatty liver complicated with dyslipidemia were closely related to dyslipidemia(P<0.05).The diagnostic model of ultrasound parameters and blood lipid in patients with fatty liver was:Logit (parameter) =1.615X1 +1.933X2 +1.526X3 +1.335X4 -2.939;The AUC of the area under the ROC curve was 0.831 (95%CI:0.729-0.934).When the ultrasonic parameters were divided into a cut-off value of 0.6, the sensitivity of the diagnostic model was 87%, and the specificity was 68.4%.There was a significant positive correlation between ultrasound score and TG, TC and ldl-c levels (r >0, P<0.05), and a significant negative correlation between ultrasound score and hdl-c levels (r<0, P< 0.05).Conclusion The ultrasonic-lipid diagnosis model established in this study can effectively combine ultrasound parameters and lipid levels in patients with fatty liver, enhance the correlation between the two, and promote early screening and diagnosis in patients with fatty liver. |
