Quantitative Assessment of left ventricular function in patients with chronic kidney diseases by Vector Flow Mapping WANG Wei,BAI Hui,ZHOU Jinling,CHANG Qing,WANG Yueheng Department of Cardiac Ultrasound,The Second Hospital of He bei Medical University,Shijiazhuang 050000,China ABSTRACT Objective To analyze the change characteristics of left ventricular energy loss(EL) in chronic kidney diseases by vector flow mapping(VFM)technique and explore the clinical value of EL in quantitative evaluating left ventricular function in patients with CKDchronic kidney diease.Methods Sixty-one CKD chronic kidney diease patients were selected in this study,including 42 patients with LVEF≥50% in group A,19 patients with LVEF<50% in group B,Thirty-eight healthy adults were enrolled as normal control group. VFM analysis was executed to acquire and calculate left ventricle average EL during early filling phase(E-EL-ave),atrial filling phase(A-EL-ave),isovolumic contraction period(I-EL-ave),and ejection phase(Ej-EL-ave),the differences of above parameters in each group were compared. Receiver operating characteristic curve was drawn to analyze the diagnostic efficacy of different degrees of left ventricular dysfunction. Then the relationship between each phases EL and conventional ultrasound parameters were analyzed. Results 1.Compared with the normal control group,EL in all phases of group A were increased. A-EL-ave increased in group B,and Ej-EL-ave was to reduce. The differences were statistically significant(P<0.05). the control group to group B,Ej-EL-ave showed a biphasic trend in the control group , group A and group B. .Ej-EL-ave has good diagnostic efficacy for early left ventricular function impairment and LVEF<50% in patients with CKD(AUC=0.907,0.861,respectively,P<0.001) 2.Ej-EL-ave were strongly associated with LVEF in the patients with chronic kidney diease(r=0.636,P<0.01).Conclusion The parameter Ej-EL-ave measured by VFM technique can be used as a potential new indicator to evaluate the efficiency of left ventricular pump function in chronic kidney disease patients, which has certain clinical application value.
