目的 探究心肌纵向应变及应变达峰时间离散度(PSD)在评价左室射血分数(LVEF)正常房颤(AF)患者的左室收缩功能中的价值。方法 选取38例LVEF正常的持续性AF患者作为LVEF正常AF组,另外选取38例健康体检者作为对照组,收集相关临床资料,分析并比较两组间左房内径(LAd)、左室舒张末期内径(LVEDd)、左室收缩末期内径(LVESd)、LVEF、左室整体纵向应变均值(GLPS-AVG)、各平面整体纵向应变均值(GLPS-4CH、GLPS-2CH、GLPS-APLAX)及PSD。结果 LVEF正常AF组LAd、LVEDd、LVESd较对照组明显增高(P<0.05),而年龄、性别、心率、LVEF较对照组无明显差异;LVEF正常AF组较对照组GLPS-AVG绝对值、GLPS-4CH绝对值、GLPS-2CH绝对值、GLPS-APLAX绝对值明显减低(P<0.05),而 LVEF正常AF组PSD较对照组明显升高(P<0.05);LVEF正常AF组和对照组GLPS-AVG绝对值均与PSD呈负相关,相关系数分别为r= -0.519,r=-0.722;GLPS-AVG、PSD测量在观察者间及观察者内具有良好的重复性。结论 LVEF正常的AF患者左心室各平面心肌应变、整体心肌应变及心肌收缩同步性均出现异常,应用心肌纵向应变及PSD可以很好的评价左室收缩功能。 |

Abstract: |
Objective To investigate the value of myocardial strain and peak strain dispersion(PSD) in the assessment of left ventricular systolic with atrial fibrillation(AF) and normal left ventricular ejection fraction(LVEF). Methods Thirty eight patients with AF and normal LVEF were selected as AF group,and thirty eight healthy volunteers were selected as control group.Left atrial diameter(LAd)、left ventricular end-diastolic diameter(LVEDd)、left ventricular end-systolic diameter(LVESd)、LVEF、left ventricular global longitudinal peak strain of average(GLPS-AVG)and(GLPS-4CH、GLPS-2CH、GLPS-APLAX)、PSD were measured and compared between the two groups.Results LAd、LVEDd、LVESd were higher in the AF group than those in the control group(P<0.05),while there was no significant difference of age、gender、heart rate、LVEF between the two groups.The absolute value of GLPS-AVG、GLPS-4CH、GLPS-2CH、GLPS-APLAX in the AF group were significantly lower than those in the control group(P<0.05),while PSD in the AF group were significantly higher than those in the control group(P<0.05).There was a negative correlation between absolute value of GLPS-AVG and PSD in AF group and control group(r=-0.519,r=-0.722).Measurement of GLPS-AVG、PSD had well repeatability in the inter-observer and intra-observer. Conclusions The Myocardial strain in each plane、Global myocardial strain、Myocardial contraction synchronization is abnormal in patients with AF.Myocardial strain and peak strain dispersion can well evaluate left ventricular systolic function. |
