目的 应用超声引导下关节腔内注射雷公藤甲素纳米粒(TPN)治疗兔类风湿关节炎,探讨超声在评估兔关节炎疗效中的应用价值。方法 选取27只新西兰大白兔建立诱导类风湿关节炎模型后,随机分为4组:对照组(6只)仅穿刺但不注射药物;TPN组、雷公藤甲素(TP)组和倍他米松(BS)组(每组各7只)分别于超声引导下兔膝关节腔内注射TPN、TP、BS,观察兔膝关节肿胀程度,应用超声评估兔膝关节滑膜厚度、滑膜血流信号及骨侵蚀情况;分析其与病理结果的相关性。结果 与对照组比较,TP组、BS组和TPN组滑膜血流的超声评分均降低,骨侵蚀的超声评分与病理评分也均较低,差异均有统计学意义(均P<0.05);与TP组比较,BS组和TPN组滑膜增生的超声评分和病理评分均较低,差异均有统计学意义(均P<0.05);TP组、BS组和TPN组滑膜血流的超声评分和骨侵蚀的严重程度比较,差异均无统计学意义。所有AIA模型的滑膜增生和骨侵蚀的超声评分与其病理评分均呈正相关(r=0.832、0.859,P<0.001)。结论 超声引导下关节腔内注射TPN治疗类风湿关节炎疗效好且副作用小,可以有效减轻关节滑膜炎,延缓骨质破坏;超声在评估兔关节炎疗效中具有重要的价值。 |

The value of ultrasound-guided intra-articular injection of triptolide nanoparticle on rabbit knee arthritis |

Objective To explore the curative effect of ultrasound-guided intra-articular injection of triptolide nanoparticle(TPN) on rabbit antigen-induced knee arthritis and the value of ultrasound in evaluating the curative effect of rabbit arthritis.Methods Twenty seven New Zealand white rabbits were randomly divided into 4 groups after AIA model were induced.The knee joints of TPN group,TP group and BS group were injected intra-articular with TPN,triptolide(TP) and betamethasone(BS) respectively guided by ultrasound,7 rabbits in each group.The control group (other 6 rabbits) were punctured but not injected with any drugs.The ultrasonic scores and pathological changes of joint were assessed in synovitis,synovial blood flow and the bone erosion.Meanwhile the correlation of ultrasonic scores and pathological scores was tested.Results Compared with the control group,the synovitis scores of other groups were lower(P<0.05).The ultrasonic synovitis scores of BS group and TPN group(both P<0.05) were lower than that of TP group,so were the pathological synovitis scores(both P<0.05).Compared with the control group,TP group,BS group and TPN group were all effective in improving synovial blood flow and the bone erosion and there were significant differences among them neither the ultrasonic scores nor the pathological scores.The ultrasonic scores of synovial blood flow in TP group,BS group and TPN group(all P<0.01) were lower than that of control group.The ultrasonic scores(all P<0.05) and pathological scores(all P<0.05) of bone erosion of other groups were also lower than that of control group.There were significant correlations between ultrasonic scores and pathological scores of synovial inflammation and bone erosion (r=0.832 and 0.859 respectively,both P<0.001).Conclusion High frequency ultrasound is a reliable imaging method to evaluate early arthritis and follow up treatment effect.Ultrasound-guided Intra-articular injection of TPN can relieve joint synovitis and delay the bone erosion,with slight side effect. |
