| 目的 总结食管闭锁声像图特点,评估产前超声在诊断食管闭锁中的准确性,以提高产前检出率。方法 对2016年11月至2020年9月在安徽省妇幼保健院产前超声诊断为食道闭锁的16例胎儿及随访结果进行总结,分析其特征性超声表现。结果 16例拟诊为食道闭锁胎儿产前超声表现:胃泡未显示8例,胃泡小5例；羊水过多7例,羊水多3例；腹围小于临床孕周11例；颈胸部囊袋征10例。囊袋征即盲端食管呈囊袋状扩张,胎儿吞咽运动时,囊袋状结构可变小直至消失。观察到囊袋征的胎儿经产后证实6例为食管闭锁、4例食管闭锁合并气管食管瘘,敏感性(62.5%),特异性(100%)。16例中13例引产；3例顺利出生,其中2例手术,1例正常。超声误诊2例(1例引产),超声诊断食管闭锁准确率达87.5%。结论 食管囊袋状扩张是食管闭锁特征性超声表现,羊水过多合并胃泡小或未显示是可疑超声征象,产前超声可动态观察食管及胃泡变化,在诊断胎儿食管闭锁中有重要价值。 |

| Objective To summarize the characteristics of esophageal atresia sonography and evaluate the accuracy of prenatal ultrasound in the diagnosis of esophageal atresia, so as to improve the prenatal detection rate. Methods sixteen fetuses with esophageal atresia diagnosed by prenatal ultrasound in Anhui Women and Child Health Care Hospital from November 2016 to September 2020 were summarized and their follow-up results were analyzed to analyze their characteristic ultrasonic manifestations. Results Prenatal ultrasound manifestations of 16 fetuses with esophageal atresia were proposed:8 cases non-visualization gastric bubble and 5 cases showed small gastric bubble. There were 7 cases of Polyhydramnios and 3 cases of amniotic fluid more. 11 cases of small abdominal circumference. 10 were shown with the dilated proximal esophageal pouch. When the fetuses swallowed, esophageal pouch was changed to be small until it disappeared.It was confirmed that 6 cases were esophageal atresia and 4 cases were esophageal atresia with tracheoesophageal fistula, with sensitivity (62.5%) and specificity (100%). Induction of labor in 13 of the 16 cases; Three cases were born successfully, of which 2 were operated on and 1 was normal. 2 cases were misdiagnosed by ultrasound (1 case induced labor), and the accuracy rate of ultrasonic diagnosis of esophageal atresia was 87.5%..Conclusion: Fetal EA has the characteristic ultrasonographic appearances． Prenatal sonography may be of important value in the diagnosis of fetal esophageal atresia． |
