目的 探讨高频超声在定量评价光动力疗法(PDT)治疗皮肤鲜红斑痣(PWS)中的应用价值。 方法 60例鲜红斑痣患者为研究对象,在治疗前后,用高频超声测量患者鲜红斑痣皮损处皮肤厚度及对称部位正常皮肤厚度并相减计算皮损增厚厚度;在多普勒模式下,根据Adler血流分级法对皮肤血流进行评价和分级;患者于同一拍摄条件下拍摄治疗前后皮肤标准化照片,进行视觉评分,将1-3分定义为起效,0分为无效。比较视觉评分起效及无效组患者的年龄、性别、BMI及治疗前皮损增厚厚度和血流评级。比较全部患者及不同治疗视觉评分组患者在治疗前后的皮损增厚厚度及血流评级变化。 结果 光动力治疗后,全部患者及视觉评分起效组治疗前后皮损增厚厚度、皮损血流分级差异有统计学意义(P<0.05)。视觉评分无效组治疗前后血流评级无统计学差异(P=0.899),而皮损增厚厚度由0.39 mm(0.21~0.79)降至0.31 mm(0.17~0.65),差异有统计学意义(P<0.001)。 结论 高频超声能较敏感地反映鲜红斑痣患者治疗前后的皮损厚度和血流量变化,可作用评估治疗效果的重要影像学指标。 |

Quantitative evaluation of photodynamic therapy for port-wine stains by high frequency ultrasound |

Abstract: |
Objective To investigate the potential application of high-frequency ultrasound in quantitative evaluation of photodynamic therapy (PDT) for port-wine stain (PWS). Methods A total of 60 patients with PWS were included in the study. High-frequency ultrasound were used to measure the thickness of the skin at the lesions (epidermis and dermis) of the PWS and the thickness of the normal skin at the corresponding locations, and the thickened thickness was calculated; In doppler mode the skin blood flow was evaluated and graded according to the Adler blood flow grade method. The patient took normalized photographs under the same shooting conditions before each treatment, and 1-3 scores were classified effective while 0 was classified invalid. Before the first treatment, the baseline including ages, genders and BMI, and the thickened thickness of skin lesions and the blood flow rating of lesions were compared between the effective group and the invalid group. The thickened thickness and blood flow of PWS lesions were compared among patients before and after treatment. Results After a photodynamic therapy, all patients and the with the effective group had significant differences in the thickened thickness of skin lesions and the blood flow rating of lesions before and after treatment (P<0.05). For some patients with visual score of 0 (invalid group), there was no statistical difference in blood flow signals before and after treatment (P = 0.899), but the thickened thickness of skin lesions decreased from 0.39mm (0.21-0.79) to 0.31mm (0.17- 0.65), the difference was statistically significant (P < 0.001). Conclusion High-frequency ultrasound can sensitively reflect changes in skin thickness and blood flow of PWS compared to traditional visual scores. |
