摘要 目的 应用二维斑点追踪成像技术(2D-STI)定量小儿重症肺炎(SP)患者右心室纵向应变,初步探讨其临床价值及相关因素。方法 选取70例SP患者以及70例年龄和体表面积相匹配的健康小儿作为对照组。所有受试者均行超声心动图及2D-STI检查,分别获取右心室结构、收缩功能参数:右心室舒张末内径(RVEDD)、三尖瓣环收缩期位移(TAPSE)、右心室做功指数(Tei)、右心室面积变化率(RVFAC)、右心室整体纵向应变(RVGLS)、游离壁整体纵向应变(FWGLS)及室间隔整体纵向应变(IVSGLS)。结果 ①与对照组相比,SP组患者心率、呼吸频率、Tei指数增加,TAPSE减低,(P均<0.05),而RVEDD和RVFAC在两组间差异无统计学意义(P均>0.05)。②与对照组相比,SP组患者RVGLS、FWGLS和IVSGLS均减低(P均<0.05)。③RVGLS、FWGLS和IVSGLS分别与年龄、TAPSE呈负相关(P均<0.05),而与心率呈正相关(P均<0.05)。结论 SP患者右心室纵向应变减低,这种变化早于右心室结构与功能,临床应用要考虑年龄和心率对其影响。 |

Evaluation value of the longitudinal strain of right ventricular in Children with Severe Pneumonia and related influencing factors |

Abstract Objective Two dimensional speckle tracking imaging (2D-STI) was used to quantify the longitudinal strain of right ventricle in children with severe pneumonia (SP), and the clinical value and related influencing factors were discussed. Methods 70 children with severe pneumonia were enrolled in the study, and 70 healthy children matched with age and body surface area were selected as the control group. Convention echocardiography and 2D-STI were performed in all the children. The parameters of right ventricular structure and systolic function were obtained: right ventricular end diastolic diameter (RVEDD)、pulmonary artery dimension(PAD)、tricuspid annular plane systolic excursion(TAPSE)、Tei index of the right ventricular、right ventricle fractional area change(RVFAC)、the global longitudinal strain of right ventricle (RVGLS), the global longitudinal strain of free wall (FWGLS) and the global longitudinal strain of interventricular septum (IVSGLS). Results ①Compared with the normal control group, heart rate、respiratory rate、Tei index increased , and TAPSE decreased in SP group (P<0.05), while there was no significant difference in RVEDD and RVFAC between the two groups(P>0.05). ②Compared with the normal control group, RVGLS、FWGLS and IVSGLS decreased in SP group(P<0.05). ③RVGLS、FWGLS and IVSGLS were negatively correlated with age and TAPSE (P<0.05), but positively correlated with heart rate (P<0.05). Conclusion The longitudinal strain of the right ventricle decreases in SP children, which is earlier than the structure and convention function of the right ventricle, and the influence of age and heart rate should be considered in clinical application. |
