目的 研究应用三维斑点追踪成像技术(3D-STI)技术,评价研究冠状动脉不同程度病变对冠心病患者的左心房容积和功能的影响的价值。方法 将确诊的81例CHD冠心病(CHD)患者根据冠状动脉造影的结果,按照Gensini评分法,依据冠状动脉狭窄的部位及程度的不同分为轻度病变组(Gensini评分<25分,31例)、中度病变组(25≤Gensini评分<50,26例)、重度病变组三组(Gensini评分≥50,24例),另选取了31例同时期就诊的就诊的健康者为对照组。应用RT3D-STI技术获得左心房各时相的容积指数[:左心房最大容积指数(LAVImax)、左心房最小容积指数(LAVImin)、左心房主动收缩前容积指数(LAVIpre)]及左心房三维整体应变参数[整体纵向应变 (GLS)、径向应变 (GRS)、圆周应变 (GCS)],计算,经体表面积表化后可得到相应的容积指数射血分数:左心房总的、被动及主动排空分数(LAEF、 LApEF、LAaEF)。另外应用RT3D-STI还可获得左心房整体应变参数:纵向应变 (GLS)、径向应变 (GRS)、圆周应变 (GCS)行组间分析比较,并分析Gensini分值与左心房容积及应变参数的相关性。结果①与对照组相比,轻度病变组GLS、GCS减低(均p<0.05),其他参数无明显差异;与对照组及轻度病变组相比,中度病变组LAVImin、LAVIp增大,LAEF、LApEF、GLS、GCS减低(均均p<0.05);与其他三组比较,重度病变组的LAVImin、LAVIp、LAVImax均增大,LAEF、LApEF、LAaEF、GLS、GCS、GRS均减低(均各项参数均p<0.05)。②LAVImin、LAVIpre与Gensini分值分值呈正相关(r=0.769、r=0.550, 均P<0.05),LAVImax无明显相关性(r=0.348,P<0.05);GCS、GLS、GRS与Gensini分值显示出负相关性(r=-0.618、r=-0.577、r=-0.549,均均P<0.05)。结论 3D-STI可以根据冠状动脉病变的不同程度有效评价CHD患者的左心房容积及功能,为CHD患者的早期干预、早期治疗提供依据。 |

Evaluation of left atrial volume and function in patients with Coronary artery lesions of different degreesby three-dimensional speckle tracing imaging |

Abstract: |
Objective To study the value of left atrium(LA) volume and function in patients with different degrees of coronary artery disease by using 3D speckle tracking imaging (3D-STI). Methods According to the results of coronary angiography, 81 patients with confirmed coronary heart disease (CHD) were divided into mild lesion group (Gensini score<25,n=31),moderate lesion group (25≤Gensini score<50,n=26),severe lesion group (Gensini score≥50,n=24),and 31 healthy subjects at the same period were selected as control group.LA volume indexs including LA maximum indexs volume (LAVImax), LA minimum indexs volume (LAVImin), LA presystolic indexs volume (LAVIpre) and three-dimensional strain parameters of LA including global longitudinal strain (GLS), global radial strain (GRS) and global circumcircular strain (GCS) were obtained by 3D-STI technique.LA ejection fraction, passive and active ejection fractions (LAEF, LApEF, LAaEF) were calculated. Analysis and comparison between row groups and the correlation between Gensini score and LA volume and strain parameters were analyzed. Results ① Compared with the control group, GLS and GCS were decreased in the mild lesion group (all P <0.05); Compared with the control group and the mild lesion group, the LAVImin and LAVIp increased, while LAEF, LApEF, GLS and GCS decreased in the moderate lesion group (all P <0.05); Compared with the other three groups, the LAVImin, LAVIp and LAVImax in the severe lesion group were increased, while LAEF, LApEF, LAaEF, GLS, GCS and GRS were decreased (all P <0.05). ② LAVImin and LAVIpre were positively correlated with Gensini score (r=0.769,r=0.550,all P<0.05);GCS, GLS, GRS showed negative correlation with Gensini scores (r=-0.618, r =-0.577, r=-0.549, all P < 0.05). Conclusions 3D-STI can effectively evaluate the LA volume and function of CHD patients with varying degrees of coronary artery lesions, providing evidence for early intervention and treatment of CHD patients. |
